INTRODUÇÃO: O estresse oxidativo está relacionado com o envelhecimento e com vários distúrbios neurodegenerativos. Os flavonoides por sua vez exibem ação importante nos sistemas biológicos, como a eliminação de radicais livres, atuando como agentes antioxidantes, seja isoladamente ou em conjunto com moléculas como por exemplo o ácido ascórbico. Flavonoides como a rutina se destacam por sua capacidade antioxidante e citoprotetora, sendo assim, promissores nesse âmbito. OBJETIVO: Investigar o efeito protetor do flavonoide rutina contra a citotoxicidade do sulfato ferroso em modelos in vitro de linhagem C6 de glioma de rato, e avaliar se o ácido ascórbico interfere nesta atividade farmacológica. MÉTODOS: As células C6 foram cultivadas em condições estéreis, a 37ºC em atmosfera com 5 % de CO2 até atingirem confluência. Foram feitas modulações específicas para cada etapa; para determinação da mínima concentração citotóxica do sulfato ferroso, a diluição ocorreu a partir de 22 mM. Foi feito teste de citotoxicidade do sulfato ferroso após 24h de tratamento através de teste de MTT. Para mínima concentração protetora da rutina foi feita diluição (a partir de 500 µM por 24h), seguido de tratamento com sulfato ferroso (a 13 mM por 24h), assim como teste de análise da proteção de células da rutina através de teste de MTT. Para verificar interferência do ácido ascórbico foi feito experimento em que as células C6 foram pré-tratadas com rutina na mínima concentração protetora (5 µM por 24 h), sendo em seguida tratadas com sulfato ferroso (a 13,7 mM), variandose a concentração de ácido ascórbico (entre 6 - 300 µM por 24h de tratamento) através de teste de MTT. RESULTADOS: A mínima concentração citotóxica do sulfato ferroso para células C6 foi 0,7 mM após 24 h. A rutina exibiu um efeito protetor parcial em testes de viabilidade celular. A mínima concentração protetora da rutina (5 µM) preveniu uma porcentagem significativa de morte celular induzida pelo sulfato ferroso a 13 mM após 24h. O ácido ascórbico não interferiu no efeito protetor da rutina. CONCLUSÕES: A rutina tem ação citoprotetora frente ao dano oxidativo oriundo do sulfato ferroso, em células C6, o ácido ascórbico não apresentou interferência nessa proteção.

INTRODUCTION: Oxidative stress is related to aging and several neurodegenerative disorders. Flavonoids, on the other hand, have an important action in biological systems, for example as scavenging free radicals, acting as antioxidant agents, either alone or together with molecules such as ascorbic acid. Flavonoids like rutin stand out for their antioxidant and cytoprotective capacity, therefore, they are promising in this field. AIM: To investigate the protective effect of the flavonoid rutin against the cytotoxicity of ferrous sulfate in in vitro models of the C6 strain of rat glioma, and to assess whether ascorbic acid interferes with this pharmacological activity. METHODS: C6 cells were cultured under sterile conditions at 37ºC in an atmosphere with 5% CO2 until reaching confluence. Specific modulations were made for each step; to determine the minimum cytotoxic concentration of ferrous sulfate, the dilution occurred from 22 mM. The ferrous sulfate cytotoxicity test was performed after 24h of treatment using the MTT test. For a minimum protective concentration of rutin, a dilution was made (from 500 µM for 24h), followed by treatment with ferrous sulfate (at 13 mM for 24h), as well as an analysis test of the protection of rutin cells through the MTT test. To verify interference from ascorbic acid, an experiment was carried out in which C6 cells were pretreated with rutin at the minimum protective concentration (5 µM for 24 h), being then treated with ferrous sulfate (at 13.7 mM), varying the concentration of ascorbic acid (between 6 - 300 µM per 24h of treatment) using the MTT test. RESULTS: The minimum cytotoxic concentration of ferrous sulfate for C6 cells was 0.7 mM after 24 h. Rutin exhibited a partial protective effect in cell viability tests. The minimum protective concentration of rutin (5 µM) prevented a significant percentage of cell death induced by ferrous sulfate at 13 mM after 24 h. Ascorbic acid did not interfere with the protective effect of rutin. CONCLUSIONS: Rutin has a cytoprotective action against oxidative damage from ferrous sulfate, in C6 cells, ascorbic acid did not interfere in this protection.

RESUMO O carcinoma escamocelular oral (CEO) é um grave problema de saúde pública com elevada prevalência em populações com baixo nível socioeconômico, sendo frequentemente diagnosticado em fase tardia, quando o prognóstico se torna reservado. Ainda há poucas opções de fármacos para o tratamento deste tumor nos pacientes. Previamente, demonstramos que a via embrionária Hedgehog (HH) está reativada neste tumor, levando a um perfil de maior agressividade. Deste modo, a busca por fármacos inibidores da via HH, seja inibindo sua ativação canônica ou não canônica, passa a ser de grande relevância para o tratamento do CEO. Dentre os mecanismos de ativação não canônica da via HH destaca-se a interação com PI3K/AKT, uma importante via de sobrevivência celular. O objetivo deste trabalho foi avaliar a atividade antitumoral in vitro e a inibição farmacológica de componentes da via HH por um inibidor farmacológico de PI3K, o Pictilisib, em linhagem metastática de CEO. Deste modo, foi realizado o tratamento das células HSC3 com Pictilisib em diferentes concentrações para avaliação da citotoxicidade, viabilidade celular, análise de apoptose por ensaio de Anexina-PI em citometria de fluxo, análise de migração celular por scratch assay, além da avaliação da expressão dos componentes da via HH por western blot, Imunofluorescência e RT-qPCR. O Pictilisib apresentou perfil de citotoxicidade na linhagem HSC3, com valor de CI50 de 0,05 µM. Nas concentrações de 0,97 μM e 1,94 μM, foi demonstrada significativa redução da viabilidade celular, além de aumento do percentual de células na fase sub-G1 após 48 e 72 h de incubação com o Pictilisib na concentração de 0,97 μM. O ensaio de Anexina-PI demonstrou indução de apoptose 48 e 72 h após incubação com o Pictilisib nas concentrações de 0,97 µM e 1,94 µM. O ensaio de migração celular demonstrou que o tratamento com Pictilisib reduziu a migração celular nos tempos de 6 e 24h. Através da técnica de Western blot, as proteínas PTCH1, SHH e GLI1 foram analisadas após o tratamento com o Pictilisib no período de 24h nas concentrações de 0,97 e 1,94 µM, mas não foi observada diferença na expressão destas proteínas com o tratamento. Através da técnica de imunofluorescência, as proteínas GLI1, GLI2 e SMO foram analisadas em relação à presença e à localização. Os resultados demonstram forte imunoexpressão dos componentes citados, havendo marcação nuclear das proteínas GLI1, GLI2 e SMO em ambas as concentrações com o Pictilisib. A expressão gênica dos componentes da via Hedgehog (GLI1, GLI2 e GLI3), foi avaliada através de RTqPCR na linhagem HSC3 após 24 h de incubação com Pictilisib 1,94 µM. Não foi observada inibição da expressão do mRNA de GLI1 e GLI2, quando células tratadas foram comparadas com o controle não tratado. No entanto, observou-se aumento do mRNA de GLI3 após incubação com Pictilisib a 1,94 µM. Em conclusão, os dados deste estudo dão suporte para um potencial papel do Pictilisib como um fármaco promissor para utilização terapêutica no CEO. O papel deste fármaco na interação do PI3K/AKT com a via HH merece aprofundamento em estudos futuros.

ABSTRACT Oral squamous cell carcinoma (OSCC) is a serious public health problem with a high prevalence in populations with low socioeconomic status, and is often diagnosed at a late stage, when the prognosis becomes poor. There are still few drug options for the treatment of this tumor in patients. Previously, we demonstrated that the embryonic Hedgehog (HH) pathway is reactivated in this tumor, leading to a more aggressive profile. Thus, the search for drugs that inhibit the HH pathway, either by inhibiting it’s canonical or non-canonical activation, becomes of great relevance for the treatment of OSCC. Among the non-canonical activation mechanisms of the HH pathway, the interaction with PI3K/AKT, an important cell survival pathway, stands out. The aim of this work was to evaluate in vitro antitumor activity and the pharmacological inhibition of components of the HH pathway by a pharmacological PI3K inhibitor, Pictilisib, in a metastatic line of OSCC. Thus, treatment of HSC3 cells with Pictilisib at different concentrations was performed to evaluate cytotoxicity, cell viability, apoptosis analysis by annexin-PI assay in flow cytometry, cell migration analysis by scratch assay, in addition to the evaluation of expression of the components of the HH pathway by Western blot, Immunofluorescence and RT-qPCR. Pictilisib presented a cytotoxicity profile in the HSC3 line, with an IC50 value of 0,05 µM. At concentrations of 0.97 μM and 1.94 μM, a significant reduction in cell viability was demonstrated, in addition to an increase in the percentage of cells in the sub-G1 phase after 48 and 72 h of incubation with Pictilisib at a concentration of 0.97 μM. The Annexin-PI assay demonstrated induction of apoptosis 48 and 72 h after incubation with Pictilisib at concentrations of 0.97 µM and 1.94 µM. The cell migration assay demonstrated that treatment with Pictilisib reduced cell migration at 6 and 24 h. Through the Western blot technique, PTCH1, SHH and GLI1 proteins were analyzed after treatment with Pictilisib in a period of 24h at concentrations of 0.97 and 1.94 µM, but no difference was observed in the expression of these proteins with the treatment . Using the immunofluorescence technique, the proteins GLI1, GLI2 and SMO were analyzed in relation to their presence and location. The results demonstrate strong immunoexpression of the mentioned components, with nuclear labeling of proteins GLI1, GLI2 and SMO at both concentrations with Pictilisib. The gene expression of the components of the Hedgehog pathway (GLI1, GLI2 and GLI3) was evaluated by RT-qPCR in the HSC3 line after 24 h of incubation with 1.94 µM Pictilisib. No inhibition of GLI1 and GLI2 mRNA expression was observed when treated cells were compared with untreated control. However, an increase in GLI3 mRNA was observed after incubation with Pictilisib at 1.94 µM. In conclusion, the data from this study support a potential role of Pictilisib as a promising drug for therapeutic use in OSCC. The role of this drug in the interaction of PI3K/AKT with the HH pathway deserves further investigation in future studies.

RESUMO INTRODUÇÃO: A leishmaniose é um conjunto de doenças negligenciadas, e uma das suas formas mais frequente no Brasil é a leishmaniose cutânea causada Leishmania braziliensis (L. braziliensis). Esta forma clínica é caracterizada por lesões cutâneas únicas na forma de úlcera com bordas elevadas e fundo necrótico, causada principalmente pela resposta inflamatória exacerbada, induzida por células Th1 produtoras de IFN-γ e apresentando um número reduzido de parasitas no local da lesão. Em modelos de doenças autoimunes e inflamatórias, o uso da indução de tolerância oral (TO) tem se mostrado promissor no desenvolvimento de estratégias terapêuticas, pois os mecanismos regulatórios gerados pela TO diminuem a inflamação causada nestas doenças. Trabalhos da literatura têm mostrado que as Lactococcus lactis (L. lactis), bactérias lácticas, não patogênicas e Gram-positivas são boas ferramentas para a indução da TO. Neste trabalho, utilizamos a cepa recombinante de L. lactis geneticamente modificada produtora da proteína de choque térmico 65 (Hsp65) derivada do Mycobacterium leprae. A Hsp65 possui um papel regulador importante no sistema imune, modulando principalmente respostas inflamatórias devido a sua capacidade de inibir a produção de citocinas, como TNF e IFN-γ, e de aumentar IL-10 pelas células T regulatórias (Tregs). OBJETIVO: Avaliar o potencial terapêutico da administração oral de L. lactis produtoras de Hsp65, como tratamento imunomodulador em modelo experimental de leishmaniose cutânea causada por L. braziliensis em camundongos BALB/c. MATERIAIS E MÉTODOS: Camundongos BALB/c foram desafiados na orelha com promastigotas metacíclicas de L. braziliensis. Após quatro semanas de infecção, os animais foram tratados por via oral com L. lactis produtoras ou não de Hsp65 durante quatro dias consecutivos. Nos diferentes grupos de animais avaliou-se a espessura da lesão, a carga parasitária, citocinas produzidas pelas células dos linfonodos drenantes da lesão e a frequência de células Tregs envolvidas na indução da TO durante todo o período de infecção. RESULTADOS: O acompanhamento semanal da espessura das lesões durante a infecção mostrou que, os animais tratados por via oral com L. lactis produtora de Hsp65 desenvolveu lesões menores e apresentaram uma menor destruição tecidual após o tratamento comparados aos animais dos grupos controles (Lb e Lb/Ø). O tratamento oral com Hsp65 reduziu a carga parasitária a partir de 6 semanas de infecção em relação aos animais que não receberam Hsp65. Foi observado também uma redução na produção de IFN-γ nos linfonodos drenantes da lesão dos animais tratados com Hsp65 seguido de um aumento de IL-10, evidenciando um balanço entre a produção de citocinas pro- e anti-inflamatórias. O tratamento oral com L. lactis produtora de Hsp65 também promoveu o aumento da frequência de células Tregs como CD4+CD25+Foxp3 + e CD4+LAP+ (TGF-β associado à membrana) nos linfonodos drenantes da lesão causada por L. braziliensis. CONCLUSÃO: A utilização da TO utilizando a proteína heteróloga Hsp65 apresenta boas perspectivas para seu emprego no tratamento da leishmaniose cutânea.

ABSTRACT INTRODUCTION: Leishmaniasis is a group of neglected diseases, and one of its most frequent forms in Brazil is the cutaneous leishmaniasis caused by Leishmania braziliensis (L. braziliensis). This clinical form is characterized by unique cutaneous lesions in the form of an ulcer with raised edges and necrotic background, mainly caused by an exacerbated inflammatory response, induced by IFN-γ-producing Th1 cells and presenting a reduced number of parasites at the lesion site. In models of autoimmune and inflammatory diseases, the use of oral tolerance (OT) induction has shown promise in the development of therapeutic strategies, as the regulatory mechanisms generated by OT decrease the inflammation caused in these diseases. Literature studies have shown that Lactococcus lactis (L. lactis), non-pathogenic and Gram-positive lactic bacteria are good tools for the induction of OT. In this work, we used the genetically modified recombinant strain of L. lactis producing heat shock protein 65 (Hsp65) derived from Mycobacterium leprae. Hsp65 has an important regulatory role in the immune system, mainly modulating inflammatory responses due to its ability to inhibit the production of cytokines, such as TNF and IFN-γ, and to increase IL-10 by regulatory T cells (Tregs). OBJECTIVE: To evaluate the therapeutic potential of oral administration of Hsp65- producing L. lactis as an immunomodulatory treatment in an experimental model of cutaneous leishmaniasis caused by L. braziliensis in BALB/c mice. MATERIALS AND METHODS: BALB/c mice were challenged in the ear with L. braziliensis metacyclic promastigotes. After four weeks of infection, the animals were treated orally with L. lactis producing or not producing Hsp65 for four consecutive days. In the different groups of animals, the thickness of the lesion, the parasite load, cytokines produced by the cells of the lymph nodes draining the lesion and the frequency of Tregs cells involved in the induction of TO during the entire infection period were evaluated. RESULTS: Weekly monitoring of lesion thickness during infection showed that animals treated orally with Hsp65-producing L. lactis developed smaller lesions and showed less tissue destruction after treatment compared to animals in the control groups (Lb and Lb/Ø). Oral treatment with Hsp65 reduced the parasite load from 6 weeks of infection in relation to animals that did not receive Hsp65. It was also observed a reduction in the production of IFN-γ in the lymph nodes draining the lesion of the animals treated with Hsp65 followed by an increase in IL-10, evidencing a balance between the production of proand anti-inflammatory cytokines. Oral treatment with Hsp65-producing L. lactis also increased the frequency of Treg cells such as CD4+CD25+Foxp3+ and CD4+LAP+ (membrane-associated TGF-β) in the draining lymph nodes of the lesion caused by L. braziliensis. CONCLUSION: The use of TO using the heterologous protein Hsp65 presents good prospects for its use in the treatment of cutaneous leishmaniasis.

RESUMO Introdução: A leishmaniose visceral (LV) é uma doença infecciosa causada pela Leishmania infantum, em que há o comprometimento de órgão internos. No baço a infecção é crônica e progressiva. Além disso, na LV grave ocorre a desorganização dos compartimentos esplênicos, a diminuição do número de células B na polpa branca (PB) e plasmocitose na polpa vermelha (PV). Os plasmócitos têm sobrevida estendida e acumulam-se em várias doenças crônicas. Contudo, pouco se sabe sobre as funções e os mecanismos de geração e manutenção da sobrevida dessas células. Objetivo: Neste trabalho, investigamos os potenciais mecanismos associados com o desenvolvimento anômalo de células B que levam à plasmocitose no curso da LV. Métodos: 56 hamsters da linhagem Golden Sirius com 6 a 8 semanas de idade foram usados neste estudo: 28 foram injetados intraperitonealmente com 1x107 promastigotas de L. infantum e 28 receberam a solução diluente pela mesma via. Sete animais de cada grupo foram eutasiados 30, 60, 120 e 150 dias após a injeção (dpi). Os animais foram necropsiados e amostras dos órgãos foram fixadas em parafina para estudo histológico e avaliação da distribuição de célula B e plasmócitos por imuno-histoquímica. Para três animais de cada grupo e em cada ponto, um fragmento do baço foi colhido para avaliação transcriptômica da expressão gênica e associação entre moléculas relacionadas com o desenvolvimento de células B, incluindo uma molécula chamada DLK1. Resultados: Houve aumento da PB dos animais infectados 120 dpi, enquanto, no ponto consecutivo ocorreu atrofia, após 150 dias. Afetando a região de folículo linfoide, possivelmente devido a diminuição de células B. Também observamos redução da quimiotaxia de células B, pela diminuição de CXCL13 e CXCL12 que ocorreu 120 e 150 dpi. Houve uma tendência a diminuição de fatores moleculares relacionados com a resposta de centro germinativo (CG), observado pela expressão gênica dos hamsters infectados. Não houve alterações na quantidade de células B na PV dos animais infectados, mas houve progressiva plasmocitose na PV dos animais nos pontos mais tardios da doença, 120 e 150 dpi e o acúmulo dessas células na zona de células T 120 dpi, sendo que DLK1 está associada com a plasmocitose na PV. A expressão gênica dos animais infectados confirmou a diferenciação de plasmócitos 150 dpi e associou a sinalização por IL-21 e IFN- como as principais moléculas envolvidas. Além disso, houve o aumento de IL-6 150 dpi. Conclusões: Neste estudo correlacionamos alterações estruturais e celulares com a expressão molecular, para investigar os determinantes da diferenciação anômala de células B em formas graves da LV. Os resultados deste trabalho demonstram aumento de IL-21 e INF- como moléculas responsáveis pelas alterações nas vias de diferenciação de células B, que geram plasmócitos em estágio crônico da doença. Além disso, demonstramos uma diminuição de CXCL13 e CXCL12, e um aumento de IL-6. Houve aumento da proporção de células B 120 dpi, mas diminuição em 150 dpi na PB e plasmocitose na PV.

ABSTRACT Introduction: Visceral leishmaniasis (VL) is an infectious disease caused by Leishmania infantum, which internal organs are compromised. In the spleen the infection is chronic and progressive. In addition, in severe VL, there is disruption of splenic compartments, a decrease of B cells number in the white pulp (WP) and plasmacytosis in the red pulp (RP). Plasma cells have extended survival and accumulated in many chronic diseases. However, poorly is known about the functions and mechanisms of generation and maintenance of cell survival. Objective: In this work, we investigated the potential mechanisms associated with the abnormal development of B cells that lead to plasmacytosis during VL. Methods: 56 Golden Syrian hamsters with 6 to 8 weeks were used in this study: 28 were injected intraperitoneally with 1x107 L. infantum promastigotes and 28 received the diluent solution by the same route. Seven animals from each group were euthanized 30-, 60-, 120- and 150-days post-injection (dpi). The animals were necropsied and organ samples were fixed in paraffin for histological study and evaluation of B cell and plasma cell distribution by immunohistochemistry. For three animals from each group and at each point, a spleen fragment was collected for transcriptomic evaluation of gene expression and association between molecules related to B cell development, including a molecule called DLK1. Results: There was an increase in the WP of the infected animals at 120 dpi, while at the 150 dpi there was atrophy affecting the follicle, which might be related to a decrease in B cells. We also observed a reduction in B cell chemotaxis, due to the decrease in CXCL13 and CXCL12 that occurred at 120 and 150 dpi. We noticed a toward to decrease molecular factors related to the germinal center (GC) response, observed by the gene expression of infected hamsters. There were no changes in the amount of B cells in the RP of the infected animals, but there was progressive plasmacytosis in the RP of the animals at the later stages of the disease, 120 and 150 dpi and the accumulation of these cells in the T cell zone at 120 dpi, DLK1 was associated with the plasmacytosis in RP. The gene expression of infected animals confirmed the differentiation of plasma cells at 150 dpi and associated signaling by IL-21 and IFN-γ such the main molecules involved. In addition, there was an increase in IL-6 150 dpi. Conclusions: In this study we correlated structural and cellular changes with molecular expression to investigate the determinants of B cell anomaly in severe forms of VL. There were increased of IL-21 and IFN-γ related with the B cell differentiation and plasma cell generation to latter disease stages. In addition, we demonstrated a decrease in CXCL13, CXCL12 and increase in IL-6. Moreover, there was an increase in the proportion of B cells at 120 dpi but decrease at 150 dpi in WP and plasmacytosis in RP.

RESUMO INTRODUÇÃO: Tuberculose (TB), doença infecciosa crônica causada pelo Mycobacterium tuberculosis (Mtb), é considerada um grave problema para a saúde pública. Cerca de 40% do peso seco da parede do Mtb é composta por lipídios. Grande parte do genoma bacteriano é dedicado à biossíntese e degradação dessas moléculas. O Mtb contém transportadores os quais estão envolvidos na importação desses lipídios através da parede celular que são codificados pelos operons mce 1 a 4. Em um estudo recente, foi demonstrado em modelo in vitro que o Mtb, ao reprimir a expressão do operon mce1, é capaz de modificar a composição lipídica de sua parede celular, acumulando ácidos micólicos livres. Outro estudo demonstrou que os lipídios do Mtb apresentam capacidade distinta de ativar a resposta pró-inflamatória em macrófagos murinos e em células T humanas. Assim, por serem moléculas importantes para a virulência do bacilo e por serem capazes também de estimular uma resposta imune celular no hospedeiro, nessa dissertação, exploramos se os lipídios do Mtb são capazes de estimular uma resposta humoral com a produção de imunoglobulinas (Igs) que possam ser detectadas no soro humano, em diferentes populações. OBJETIVO: Avaliar os níveis de IgG e IgM responsivos aos extratos lipídicos do Mtb, da cepa selvagem e mutante no operon mce1, além de identificar se essas Igs podem ser exploradas como possíveis marcadores de doença e infecção tuberculosa. MATERIAL E MÉTODOS: Voluntários foram convidados para participar de um estudo de corte transversal mediante a assinatura do termo de consentimento livre e esclarecido (TCLE). Os soros desses indivíduos foram coletados e armazenados a -80ºC até a sua utilização. Ensaios de ELISA foram realizados para identificar a detecção de anticorpos IgG e IgM contra os extratos lipídicos do Mtb adsorvidos nas placas. A avaliação dos níveis dessas Igs foi realizada através do índice de reatividade (IR), média geométrica e intervalo de confiança (IC) de 95%, além disso alguns parâmetros de desempenho dos testes foram avaliados como curva ROC, sensibilidade, especificidade, acurácia, dentre outros. RESULTADOS: Observa-se que as características clínicas e demográficas dos grupos selecionados para o estudo são bem similares. Os ensaios realizados demonstram que não há uma diferença significativa na dosagem dos anticorpos entre os grupos em estímulo aos extratos lipídicos do Mtb de ambas as cepas, com exceção dos níveis de IgG e IgM em estímulo aos lipídios da cepa selvagem no grupo de indivíduos com outras doenças pulmonares (ODP) em relação aos demais. CONCLUSAO: Os extratos lipídicos do Mtb apresentaram capacidade limitada em diferenciar os grupos testados no estudo pelas dosagens de IgG e IgM. Entretanto, observou-se um aumento nos títulos de anticorpos responsivos aos extratos lipídicos do Mtb no grupo ODP, o qual pode ser explorado como um teste diferencial de triagem. Além disso, mais estudos devem ser realizados buscando avaliar o papel de espécies lipídicas isoladas do Mtb como possíveis marcadores de prognóstico ou diagnóstico da doença, como forma de aumentar a sensibilidade e especificidade dos testes.

ABSTRACT INTRODUCTION: Tuberculosis (TB), a chronic infectious disease caused by Mycobacterium tuberculosis (Mtb), is considered a serious public health problem. About 40% of the dry weight of the Mtb wall is composed of lipids. Much of the bacterial genome is dedicated to the biosynthesis and degradation of these molecules. Mtb contains transporters which are involved in the import of these lipids through the cell wall that are encoded by the mce 1 to 4 operons. In a recent study, it was demonstrated in an in vitro model that Mtb, by repressing the expression of the mce1 operon, is capable of modifying the lipid composition of its cell wall accumulating free mycolic acids. Another study demonstrated that Mtb lipids have a distinct ability to activate the pro-inflammatory response in murine macrophages and in human T cells. Thus, as they are important molecules for the virulence of the bacillus and because they are also capable of stimulating a cellular immune response in the host, in this dissertation, we explore whether Mtb lipids are capable of stimulating a humoral response with the production of immunoglobulins (Igs) that can be detected in human serum in different populations. AIM: To evaluate the levels of IgG and IgM responsive to lipid extracts of Mtb virulent wild-type strain and the mutant in the mce1 operon, in addition to identifying whether these Igs can be explored as possible markers of tuberculosis disease and infection. MATERIAL AND METHODS: Volunteers were invited to participate in a cross-sectional study by signing an informed consent form (ICF). Sera from these individuals were collected and stored at -80ºC until use. ELISA assays were performed to identify the detection of IgG and IgM antibodies against the lipid extracts of Mtb adsorbed on plates. The evaluation of the levels of these Igs was carried out through the reactivity index (RI), geometric mean and confidence interval (CI) of 95%, in addition some performance parameters of the tests were evaluated as ROC curve, sensitivity, specificity, accuracy, among others. RESULTS: It is observed that the clinical and demographic characteristics of the groups selected for the study are very similar. The tests carried out demonstrate that there is no significant difference in the antibody dosage between the groups stimulated by Mtb lipid extracts of both strains, with the exception of IgG and IgM levels stimulated by the lipids of the wild type, in the group of individuals with other pulmonary diseases (OPD) in relation to the others. CONCLUSION: The lipid extracts of Mtb showed limited ability to differentiate the groups tested in the study by the dosages of IgG and IgM. However, an increase in antibody titers responsive to Mtb lipid extracts was observed in the ODP group, which can be explored as a differential screening test. In addition, further studies should be carried out seeking to assess the role of lipid species isolated from Mtb as possible markers of prognosis or diagnosis of the disease, as a way of increasing the sensitivity and specificity of the tests.

RESUMO INTRODUÇÃO: O vírus Dengue (DENV) já circula no Brasil a aproximadamente 35 anos, no ano de 2014, o vírus Chikungunya (CHIKV) foi identificado pela primeira vez no país, sucedido pela introdução do vírus Zika (ZIKV) no ano seguinte. A circulação dessas 3 arboviroses pelo território brasileiro, resultou em grandes epidemias que atingiram com maior força as regiões Sudeste e Nordeste do país, além dos casos agudos, graves sequelas dessas infecções como artralgia crônica e microcefalia, contribuíram para o entendimento de que neste cenário epidêmico de alta complexidade o desenvolvimento de pesquisas e reforço na vigilância eram não só necessários como também urgentes. Apesar dos esforços da comunidade cientifica, inquéritos que visam avaliar o impacto e susceptibilidade de populações a estes arbovírus ainda são escassos na literatura, principalmente em regiões rurais do país afastadas dos grandes centros urbanos. OBJETIVO: Estimar a prevalência de positividade para anticorpos específicos marcadores de infecções prévias pelo vírus Zika, vírus Dengue e vírus Chikungunya, em residentes de áreas urbanas e rurais de um município no estado da Bahia, afastado da capital Salvador e sua região metropolitana. METODOLOGIA: Este é um estudo de corte-transversal realizado no município de Conde-BA, em três vilarejos rurais e em áreas urbanas próximas ao centro da cidade, nas quais foram incluídos indivíduos com idade a partir de 4 anos dos quais foram coletadas amostras de soro sanguíneo. Essas amostras foram utilizadas para a realização do ELISA para detecção de anticorpos anti-DENV, anti-ZIKV e anti-CHIKV. Todas as amostras com resultado reagente no ELISA anti-ZIKV, foram submetidas a um PRNT, com endpoint de 90% (PRNT90). RESULTADOS: No total foram avaliadas 328 amostras. Do total 144 (43,9%) soropositivas para DENV, 57 (17,4%) para Zika e 18 (5,5%) soropositivas para CHIKV. Dentre as soropositivas 118 (77,6%) amostras eram provenientes de áreas urbanas do município e 109 (51,7%) amostras provinham de participantes do sexo feminino. Na faixa etária entre 30 e 59 anos foi observada a maior frequência de positividade, nesta mesma faixa etária, observa-se uma associação com soropositividade para arbovírus principalmente dentre os residentes da zona rural (RP: 6,86; IC95%: 2,16-21,78), uma associação positiva é também observada dentre os participantes com renda superior a 1 salário mínimo (RP: 1,44; IC95%: 1,12-1,84). Após o ajuste do modelo de regressão, observa-se associação com soropositividade apenas dentre os indivíduos com renda familiar mensal, maior que 1 salário mínimo (RP: 1,30 IC95%: 1,02-1,65) e os participantes de raça não negra (RP: 1,31; IC95%: 1,00-1,69). CONCLUSÃO: Os dados avaliados apontam que a exposição prévia ao DENV e ao CHIKV na população residente em setores urbanos do município se assemelha a encontrada em outros estudos desenvolvidos inclusive em grandes centros urbanos. É possível observar também que provavelmente ainda não houve circulação expressiva de ZIKV e CHIKV nas localidades rurais estudadas, o que aponta estes indivíduos como suscetíveis a futuras epidemias. A identificação de susceptibilidade é um fator importante para implementação de medidas de prevenção e controle, e fomenta o desenvolvimento de estudos nesta e em outras regiões com características semelhantes.

ABSTRACT INTRODUCTION: The Dengue virus (DENV) has been circulating in Brazil for approximately 35 years, in the year 2014, the Chikungunya virus (CHIKV) was first identified in the country, succeeded by the introduction of the Zika virus (ZIKV) the following year. The circulation of these 3 arboviruses through the Brazilian territory resulted in major epidemics that hit with greater force the Southeast and Northeast regions of the country, in addition to acute cases, severe sequelae of these infections such as chronic arthralgia and microcephaly, contributed to the understanding that in this epidemic scenario of high complexity the development of research and strengthening in the surveillance was not only necessary but also urgent. Despite the efforts of the scientific community, surveys aimed at evaluating the impact and susceptibility of populations to these arboviruses are still scarce in the literature, especially in rural regions of the country far from large urban centers. OBJECTIVE: To estimate the prevalence of positivity for specific antibodies markers of previous infections by Zika virus, Dengue virus, and Chikungunya virus, in residents of urban and rural areas of a municipality in the state of Bahia, far from the capital Salvador and its metropolitan region. METHODOLOGY: This is a cross-sectional study conducted in the municipality of Conde-BA, in three rural villages and urban areas near the city center, in which individuals aged 4 years and older were included from whom blood serum samples were collected. These samples were used to perform ELISA to detect anti-DENV, anti-ZIKV, and anti-CHIKV antibodies. All samples that tested positive on the anti-ZIKV ELISA were submitted to a PRNT, with an endpoint of 90% (PRNT90). RESULTS: A total of 328 samples were evaluated. Of the total 144 (43.9%) seropositive for DENV, 57 (17.4%) for Zika, and 18 (5.5%) seropositive for CHIKV. Among the seropositive samples, 118 (77.6%) were from urban areas of the municipality and 109 (51.7%) samples were from female participants. In the age group between 30 and 59 years, the highest frequency of positivity was observed, in this same age group, an association with seropositivity for arbovirus was observed mainly among residents of rural areas (PR: 6.86; 95%CI: 2.16-21.78), a positive association is also observed among participants with income over 1 minimum wage (PR: 1.44; 95%CI: 1.12-1.84). After adjustment of the regression model, an association with seropositivity is observed only among individuals with monthly family income greater than 1 minimum wage (PR: 1.30 95%CI: 1.02-1.65) and non-black participants (PR: 1.31; 95%CI: 1.00-1.69). CONCLUSION: The evaluated data indicate that previous exposure to DENV and CHIKV in the population living in urban sectors of the city is similar to that found in other studies developed even in large urban centers. It is also possible to observe that there has probably not yet been expressive circulation of ZIKV and CHIKV in the rural areas studied, which indicates these individuals as susceptible to future epidemics. The identification of susceptibility is an important factor for the implementation of prevention and control measures and encourages the development of studies in this and other regions with similar characteristics.

RESUMO INTRODUÇÃO: A produção de uma vacina contra a esquistossomose seria uma adição valiosa as medidas atualmente em uso para controlar e, em última análise, erradicar esta doença. No entanto, até o momento nenhuma formulação vacinal avançou ao estágio final de desenvolvimento. As primeiras iniciativas se basearam na utilização de antígenos únicos que apesar de muito imunogênicos apresentavam localização citosólica ou no citoesqueleto, portanto pouco acessíveis em vermes íntegros. Esforços mais recentes avaliaram alvos da interface parasita-hospedeiro, porém sempre de maneira isolada, estratégia pouco efetiva para um parasito tão complexo. Nesse contexto, defendemos a utilização de múltiplos epítopos imunogênicos, localizados na interface parasita-hospedeiro e associados a resposta protetora contra o esquistossoma. Trabalhos realizados em áreas endêmicas mostraram modificações nos níveis e classes de anticorpos após o tratamento com Praziquantel (PZQ), que são associadas a resistência a reinfecção (RR). Este fenômeno é conhecido como “resistência induzida por droga”. A hipótese deste estudo é que seria possível explorar a resposta nestes indivíduos visando identificar epítopos associados a resistência para o desenho racional de uma vacina contra a esquistossomose. OBJETIVOS: Mapear os epítopos proteicos associados a proteção reconhecidos pelo soro de indivíduos RR ao S. mansoni de uma região endêmica da Bahia. MÉTODOS: Para identificação dos epítopos foram realizados ensaios com microarranjos de peptídeos contendo 59 proteinas de interesse utilizando o soro de indivíduos de uma área endêmica para esquistossomose na Bahia, selecionados por sua história de resistência (RR) ou susceptibilidade à reinfecção (SR) pelo S. mansoni após tratamento com PZQ. Os indivíduos foram acompanhados durante 18 meses após o tratamento com PZQ, com coletas de amostras de soro para o ELISA (IgG1, IgG4 e IgE) e de fezes para o Kato-Katz em 0, 1, 6, 12 e 18 meses. RESULTADOS: Os indivíduos RR apresentaram níveis mais elevados de anticorpos IgG1 contra membranas do tegumento (SmTeg), assim como IgE total e IgE especifico contra proteínas solúveis de vermes adultos (SWAP). Dois alvos de tegumento (Sm25 e ADP-Ribosyl Cyclase) revelaram epitopos preferencialmente reconhecidos pelo soro dos indivíduos RR. Também foram identificados epitopos com maior reatividade nos indivíduos SR nas proteínas MEG-12 e MEG-4.1 da glândula esofágica. CONCLUSÃO: Apesar da variação de reconhecimento e do compartilhamento de alvos entre os grupos (RR e SR), humanos resistentes à reinfecção por S. mansoni reconhecem preferencialmente epítopos nas proteínas (Sm25 e ADP-Ribosil ciclase). Adicionalmente, o título de anticorpos que o indivíduo apresenta parece ser tão importante quanto a natureza da proteína alvo para montar uma resposta efetiva contra o verme.

ABSTRACT INTRODUCTION: The production of a vaccine against schistosomiasis would be a valuable addition to the measures currently in use to control and, ultimately, eradicate this disease. However, no vaccine formulation has advanced to the final stage of development. The first initiatives were based on the use of unique antigens that were very immunogenic, but with cytosolic or cytoskeletal localization, therefore not exposed in live. Recently, efforts have evaluated targets at the host-parasite interface, however always as unique targets, a strategy not effective for such a complex parasite. In this context, we advocate the use of multiple immunogenic epitopes, located at the parasite-host interface and associated with a protective response against schistosomes. Endemic area studies showed changes in antibody levels and classes after treatment with Praziquantel (PZQ), which are associated with resistance to reinfection (RR). This phenomenon is known as “drug-induced resistance”. Our working hypothesis is that it would be possible to explore the response in these individuals in order to identify epitopes associated with resistance for the rational design of a schistosoma vaccine. OBJECTIVES: Pinpoint protein epitopes recognized by the serum of individuals RR to S. mansoni from an endemic region of Bahia. METHODS: To identify the epitopes, we performed peptide microarray assays containing 59 proteins of interest using the serum of individuals from an endemic area for schistosomiasis (Conde-BA), selected by their history of resistance (RR) or susceptibility (SR) to S. mansoni reinfection after PZQ therapy. Subjects were followed up for 18 months after PZQ treatment, with sampling of serum for ELISA (IgG1, IgG4 and IgE) and stool for Kato-Katz at 0, 1, 6, 12, 18 months. RESULTS: The RR individuals presented higher levels of IgG1 antibodies against tegument membranes (SmTeg), as well as total IgE and specific IgE against soluble proteins of adult worms (SWAP). Two tegument targets (Sm25 and ADP-Ribosyl Cyclase) revealed epitopes preferentially recognized by the serum of RR individuals. Epitopes with higher reactivity in SR individuals were identified in the MEG-12 and MEG-4.1 proteins of the esophageal gland. CONCLUSION: Despite the variation in epitope recognition and targets shared between groups (RR and SR), humans resistant to S. mansoni reinfection preferentially recognize epitopes in proteins (Sm25 and ADP-Ribosyl cyclase). Additionally, the antibody titer presented by the subject appears to be as important as the nature of the target protein to mount an effective response against the worm.

RESUMO INTRODUÇÃO: Os macrófagos são importantes células em processos inflamatórios e a sua desregulação pode afetar diversas doenças, tais como doenças autoimunes e doenças parasitárias. As leishmanioses, um conjunto de doenças tropicais negligenciadas, estão incluídas nesta categoria de enfermidades. A leishmaniose cutânea é caracterizada por úlceras em área de tegumento e a sua gravidade está relacionada com a imunopatogênese. O tratamento disponível é citotóxico, está associado à falha terapêutica, resistência parasitária, alto custo e tempo de administração prolongado. A fisalina F possui propriedades farmacológicas promissoras com potente atividade antileishmania e imunomoduladora em camundongos. OBJETIVO: Este trabalho teve como objetivo avaliar o papel imunomodulador em macrófagos humanos e a atividade anti-Leishmania braziliensis da fisalina F, in vitro. MATERIAL E MÉTODOS: Macrófagos humanos foram obtidos de PBMC de doadores sadios. A dosagem de citocinas inflamatórias foi realizada pelo método de ELISA no sobrenadante de macrófagos humanos estimulados com LPS e com adição ou não de fisalina F e dexametasona. A citotoxicidade da fisalina F em macrófagos humanos e a viabilidade de promastigotas de L. braziliensis foram mensuradas utilizando o Alamar Blue®. A ação da fisalina F em macrófagos humanos foi determinada após 24 horas de infecção com L. braziliensis. Ensaios de citometria de fluxo e de microscopia eletrônica foram realizados para avaliação dos possíveis mecanismos de ação da fisalina F em promastigotas de L. braziliensis. RESULTADOS: A fisalina F reduziu a produção das citocinas pró-inflamatórias IL-6, IL-1β e TNF-α por macrófagos ativados. Nos ensaios de citotoxicidade, apresentou o valor de CC50 de 6,07 ± 1,17 μM. Este composto também inibiu a proliferação de formas promastigotas de L. braziliensis, com valor de IC50 de 10,85 ± 0,99 μM e reduziu o número de macrófagos infectados e o número de amastigotas por macrófago, quando comparado com os controles. Em promastigotas, o surgimento de inclusões lipídicas, encurtamento do corpo celular, a destruição e o afrouxamento de membrana foram observados na análise ultraestrutural por MEV e MET após o tratamento com fisalina F. Análises de citometria de fluxo indicam que a fisalina F induz um mecanismo de morte similar a apoptose em L. braziliensis. CONCLUSÕES: A fisalina F inibiu a ativação de macrófagos humanos e promoveu uma atividade anti- L. braziliensis eficaz em promastigotas e em amastigotas.

ABSTRACT INTRODUCTION: Macrophages are important cells in inflammatory processes, and their deregulation can affect several diseases, such as autoimmune diseases and parasitic diseases. Leishmaniasis, a group of neglected tropical diseases, is included in this category of diseases. Cutaneous leishmaniasis is characterized by ulcers in the tegument area and its severity is related to immunopathogenesis. The available treatment is cytotoxic, associated with therapeutic failure, parasite resistance, high cost and prolonged administration time. Physalin F has promising pharmacological properties with potent antileishmanial and immunomodulatory activity in mice. OBJECTIVE: This study aimed to evaluate the immunomodulatory role in human macrophages and the anti-Leishmania braziliensis activity of physalin F in vitro. MATERIAL AND METHODS: Human macrophages were obtained from PBMC from healthy donors. The dosage of inflammatory cytokines was performed by the ELISA method in the supernatant of human macrophages stimulated with LPS and with or without the addition of physalin F and dexamethasone. The cytotoxicity of physalin F on human macrophages and the viability of L. braziliensis promastigotes were measured using Alamar Blue®. The action of physalin F on human macrophages was determined after 24 hours of infection with L. braziliensis. Flow cytometry and electron microscopy assays were performed to evaluate the possible mechanisms of action of physalin F in L. braziliensis promastigotes. RESULTS: Physalin F reduced the production of the pro-inflammatory cytokines IL-6, IL-1β and TNF-α by activated macrophages. In the cytotoxicity assays, it presented a CC50 value of 6.07 ± 1.17 μM. This compound also inhibited the proliferation of promastigote forms of L. braziliensis, with an IC50 value of 10.85 ± 0.99 μM, and reduced the number of infected human macrophages and the number of amastigotes per macrophage, when compared to controls. In promastigotes, the appearance of lipid inclusions, rounding of the cell body, destruction and loosening of the cell membrane were observed in the ultrastructural analysis by MEV and MET after treatment with physalin F. Flow cytometry analyzes indicate that physalin F induces apoptosis in L. braziliensis. CONCLUSIONS: Physalin F inhibited the activation of human macrophages and promoted an effective anti-L. braziliensis activity, in both promastigotes and amastigotes.

RESUMO INTRODUÇÃO: O HIV é um vírus de infecção de progressão lenta que se não tratado, gera um estado de imunossupressão que pode alterar o perfil de resposta imune de memória protetora a infecções subsequentes por outros vírus. Em indivíduos sadios, a infecção por CHIKV apresenta alta morbidade e com desenvolvimento de um quadro crônico de artralgia e artrite que pode durar mais de um ano em mais de 50% dos infectados. Em um contexto de co-circulação de HIV-1 e outras infecções, é fundamental compreender como a infecção secundária por arboviroses evoluem em pessoas que vivem com HIV/AIDS (PVHA) em tratamento com antirretrovirais e se estes indivíduos são capazes de produzir uma resposta imune protetora e duradoura a estes microrganismos. OBJETIVO: Assim, o objetivo deste trabalho foi avaliar o perfil quali-quantitativo da resposta imune celular de memória de pessoas que vivem com HIV/AIDS com história de infecção por CHIKV atendidos no Centro de Referência DST/HIV/AIDS de Feira de Santana, Bahia. MATERIAIS E METÓDOS: Foram recrutados 634 PVHA para participarem de um estudo de soroprevalência de infecção prévia ao vírus do CHIKV através da detecção de anticorpos IgG anti-CHIKV. Destes, foram convidados para participar de uma recoleta 46 indivíduos que apresentaram IgG positivo anti-CHIKV e 67 indivíduos com IgG negativo anti-CHIKV, pareados por dados clínicos e sociodemográficos. A partir disto, foi coletado 10mL de sangue total dos indivíduos selecionados e foi submetido a estímulo in vitro com 1000 PFU de CHIKV. Após 48h, o sobrenadante foi utilizado para a dosagem dos seguintes marcadores solúveis: CCL2/MCP-1, CCL3/MIP-1α, CCL4/MIP-1β, CXCL8/IL-8, CXCL10/IP-10, EGF, Eotaxina, G-CSF, GM-CSF, IFN-α2, IFN-γ, IL-10, IL-12p40, IL-12p70, IL-13, IL-15, IL-17A, IL-1Rα, IL-1α, IL-1β, IL-2, IL-3, IL-4, IL-5, IL-6, IL-7, TNF-α, TNF-β, VEGF, utilizando a tecnologia Luminex. RESULTADOS: Foi observado concentrações significativamente mais baixas de EGF e mais altas de GM-CSF, IL-8, IP-10, MIP-1α, MIP-1β, IFN-α2, IFN-γ, IL-1Rα, IL-1α, e IL-2 em PVHA com sorologia positiva para CHIKV quando comparado ao grupo PVHA com sorologia negativa para CHIKV. Em seguida, avaliamos se as quantificações das citocinas moduladas significativamente no grupo com sorologia positiva para CHIKV eram impactadas por níveis de carga viral do HIV-1, células T CD4+, CD8+, CD4+/CD8+ e CD45+ no momento da coleta. Observou-se que o nível de células T CD4+ apresentou associação significativa positiva com IFN-γ, IL1-RA, IL-2, o nível de células T CD8+ apresentou associação significativa positiva com EGF, a razão de células T CD4+/CD8+ apresentou associação significativa positiva com IP-10, IFN- α2, IFN-γ e o nível de células T CD45+ apresentou associação significativa positiva com IL-1Rα. CONCLUSÃO: Assim, concluímos que os PVHA sob tratamento de antirretrovirais são capazes de desenvolver uma resposta imune celular protetora e duradoura do tipo Th1, caracterizado principalmente por IFN-γ, IL-2, IFN- α2 e IP-10, EGF e IL1-Rα. Contudo, estas citocinas são impactadas diretamente pelo estágio de recuperação dos níveis dos leucócitos.

ABSTRACT INTRODUCTION: HIV is a slowly progressing infection virus that, if left untreated, generates a state of immunosuppression that can alter the immune response profile of protective memory to subsequent infections by other viruses. In healthy individuals, CHIKV infection has high morbidity and the development of a chronic condition of arthralgia and arthritis that can last more than a year in more than 50% of those infected. In a context of co-circulation of HIV-1 and other infections, it is essential to understand how the secondary infection by arboviruses evolves in people living with HIV/AIDS (PLWHA) in treatment with athyretrovirals and if these individuals are able to produce a response protective and lasting immune system to these microorganisms. OBJECTIVE: Thus, the objective of this study was to evaluate the qualitative-quantitative profile of the memory cellular immune response of people living with HIV/AIDS with a past history of CHIKV infection treated at the STD/HIV/AIDS Reference Center in Feira de Santana, Bahia. MATERIALS AND METHODS: A total of 634 PLWHA were recruited to participate in a study of seroprevalence of previous infection with the CHIKV virus through the detection of IgG anti-CHIKV antibodies. Of these, 46 individuals with positive anti-CHIKV IgG and 67 individuals with negative anti-CHIKV IgG were invited to participate in a recollection, matched by clinical and sociodemographic data. From this, 10mL of blood was collected from selected individuals and submitted to in vitro stimulation with 1000 PFU of CHIKV. After 48h, the supernatant was used to measure the following soluble markers: CCL2/MCP-1, CCL3/MIP-1α, CCL4/MIP-1β, CXCL8/IL-8, CXCL10/IP-10, EGF, Eotaxin, G -CSF, GM-CSF, IFN-α2, IFN-γ, IL-10, IL-12p40, IL-12p70, IL-13, IL-15, IL-17A, IL-1RA, IL-1α, IL-1β, IL-2, IL-3, IL-4, IL-5, IL-6, IL-7, TNF-α, TNF-β, VEGF, using Luminex technology. RESULTS: Significantly lower concentrations of EGF and higher concentrations of GM-CSF, IL-8, IP-10, MIP-1α, MIP-1β, IFN-α2, IFN-γ, IL-1RA, IL-1α, and IL-2 were observed in PLWHA with positive serology for CHIKV when compared to the PLWHA group with negative serology for CHIKV. Then, we evaluated if the significantly modulated cytokine quantifications in the group with positive serology for CHIKV were impacted by HIV-1 viral load levels, CD4+, CD8+, CD4+/CD8+ and CD45+ T cells at the time of collection. It was observed that the level of CD4+ T cells showed a significant positive association with IFN-γ, IL1-RA, IL-2, the level of CD8+ T cells showed a significant positive association with EGF, the CD4+/CD8+ T cell ratio showed a positive significant association with IP-10, IFN-α2, IFN-γ and the level of CD45+ T cells showed a significant positive association with IL-1RA. CONCLUSIONS: Thus, we conclude that PLWHA under antiretroviral treatment are capable of developing a protective and long-lasting Th1-type cellular immune response, characterized mainly by IFN-γ, IL-2, IFN-α2 and IP-10, EGF and IL1-Rα. However, these cytokines are directly impacted by the recovery stage of leukocyte levels.

INTRODUÇÃO: As MSCs se tornaram alvos de estudos clínicos e pré-clínicos, pelo seu potencial anti-inflamatório e imunoregulador. Sabe-se que parte dos efeitos das MSCs estão relacionados ao seu secretoma, que inclui fatores solúveis, tais como citocinas, proteínas e a liberação de vesículas extracelulares (EVs). As EVs são pequenos componentes celulares, que tem como composição moléculas bioativas (pequenos RNAs, proteínas, entre outras) que foram herdadas das células de origem e desenvolvem ações imunomoduladoras e tróficas. A análise de parâmetros que envolvem o perfil imunomodulador das EVs derivadas de MSCs (MSC-EV) é imprescindível, para ensaios importantes para o desenvolvimento clínico. OBJETIVO: Caracterizar a atividade imunomoduladora in- vitro das hucMSC-EVs em um ensaio de linfoproliferação. MATERIAL E MÉTODOS: As MSC derivadas da geleia de Wharton humana (hucMSC) foram utilizadas para o isolamento e purificação de hucMSC-EVs. As hucMSC foram caracterizadas de acordo com a ISCT analisando morfologia, imunofenotipagem e ensaios de diferenciação. Por RT-PCR avaliamos a expressão de genes relacionados com a biogênese das EVs (CD81, CD63) e ao potencial imunomodulador (IDO1 e TNFAIP6). As hucMSC-EVs foram isoladas em passagem 5 e caracterizadas seguindo as recomendações da ISEV, por microscopia eletrônica de transmissão (TEM), análise de nanofluxo e análise de rastreamento de nanopartículas (NTA). As células mononucleares do sangue (PBMC) de doadores saudáveis foram estimulados com beads anti-CD3/ anti-CD28 e incubadas com diferentes concentrações de hucMSC-EVs (10,25 e 50 μg/ml) por 72 horas a 37 °C com 5% de CO2, a análise de linfoproliferação foi feita por ensaio de ATP por luminescência. As células T reguladoras (Treg-CD4+CD25+FoxP3+) foram avaliadas por meio de citometria de fluxo. As citocinas do sobrenadante foram quantificadas por ELISA. RESULTADOS: As hucMSC cultivadas apresentaram morfologia fibroblastóide, expressão > 95% para CD90, CD105, CD44, e CD73, com <0,05% de marcadores hematopoiéticos e capacidade de diferenciação em adipócitos, osteócitos e condroblastos. A privação de lisado de plaquetas humano foi associada ao pico na expressão gênica de CD81, CD63 e TNFAIP6 após 24h, 48h e 72h. A expressão IDO1 não foi detectada. Observamos que a estabilidade do CM permanece quando armazenado a -20°C por 7 dias. As hucMSC-EVs apresentaram diâmetro médio <200 nm, mostraram morfologia típica em TEM e a análise fenotípica demonstrou marcadores positivos para EVs (Annexin, CD81, CD63) e para origem hucMSC (CD90). O tratamento com hucMSC-EVs inibiu a linfoproliferação in vitro em todas as concentrações testadas. A estimulação de Tregs foi ligeiramente aumentada após o tratamento com 50 μg/ml de MSC-EVs. A produção de TNF-a, IFN-y, e IL-10 foi inibida pela incubação com MSC-EVs, enquanto que a IL-6 não apresentou alterações significativas. CONCLUSÃO: As hucMSC-EVs apresentaram efeitos imunoregulatórios, por meio da inibição de linfócitos in-vitro, indução moderada de células Treg, porém testes adicionais precisam ser realizados, para elucidar maiores efeitos na aplicação das hucMSC-EVs em larga escala.

INTRODUCTION: MSCs have become targets of clinical and preclinical studies due to their anti-inflammatory and immunoregulatory potential. It is known that part of the effects of MSCs are related to their secretome, which includes soluble factors such as cytokines, proteins and the release of extracellular vesicles (EVs). EVs are small cellular components, which have bioactive molecules (small RNAs, proteins, among others) that were inherited from the cells of origin and develop immunomodulatory and trophic actions. The analysis of parameters involving the immunomodulatory profile of MSC-derived EVs (MSC-EV) is essential for important assays for clinical development. OBJECTIVE: To characterize the in vitro immunomodulatory activity of hucMSC-EVs in a lymphoproliferation assay. MATERIAL AND METHODS: Human Wharton's jelly-derived MSCs (hucMSCs) were used for the isolation and purification of hucMSC-EVs. The hucMSCs were characterized according to ISCT by analyzing morphology, immunophenotyping and differentiation assays. By RT-PCR we evaluated the expression of genes related to EVs biogenesis (CD81, CD63) and immunomodulatory potential (IDO1 and TNFAIP6). HucMSC-EVs were isolated in passage 5 and characterized following ISEV recommendations,by transmission electron microscopy (TEM), nanoflow analysis and nanoparticle tracking analysis (NTA). Blood mononuclear cells (PBMC) from healthy donors were stimulated with anti-CD3/ anti-CD28 beads and incubated with different concentrations of hucMSC-EVs (10,25 and 50 μg/ml) for 72 hours at 37 °C with 5% CO2, lymphoproliferation analysis was performed by luminescence ATP assay. Regulatory T cells (Treg-CD4+CD25+FoxP3+) were evaluated by flow cytometry. Cytokines in the supernatant were quantified by ELISA. RESULTS: Cultured hucMSCs showed fibroblastoid morphology, expression > 95% for CD90, CD105, CD44, and CD73, with <0.05% of hematopoietic markers, and the ability to differentiate into adipocytes, osteocytes, and chondroblasts. Deprivation of human platelet lysate was associated with a peak in gene expression of CD81, CD63 and TNFAIP6 after 24h, 48h and 72h. IDO1 expression was not detected. We observed that CM stability remains when stored at -20°C for 7 days. The hucMSC-EVs showed an average diameter <200 nm, showed typical morphology in TEM and phenotypic analysis demonstrated positive markers for EVs (Annexin, CD81, CD63) and for hucMSC origin (CD90). Treatment with hucMSC-EVs inhibited lymphoproliferation in vitro at all concentrations tested. Stimulation of Tregs was slightly increased after treatment with 50 μg/ml MSC-EVs. The production of TNF-a, IFN-y, and IL-10 was inhibited by incubation with MSC-EVs, whereas IL-6 showed no significant changes. CONCLUSION: HucMSC-EVs showed immunoregulatory effects through in-vitro lymphocyte inhibition and moderate Treg cell induction, but additional tests need to be performed to elucidate further effects on large-scale application of hucMSC-EVs.

INTRODUÇÃO: O transtorno do espectro autista (TEA) se relaciona com padrões de comportamento, estereotipias, déficits cognitivos e sociais. O tráfego endossomal e autofágico garantem o fluxo dinâmico de cargas citoplasmáticas e ambas as vias atuam de forma complementar, mas por mecanismos diferentes, evitando acúmulos de proteínas e organelas danificadas. O complexo de Golgi é uma interseção entre diversas vias intracelulares convergentes onde as proteínas Rab identificam e direcionam os compartimentos ao longo do tráfego intracelular. A proteína VPS13B está associada à membrana do Complexo de Golgi de forma interdependente às funções e estrutura. A proteína VPS13B transporta fosfolipídios diretamente entre endossomos, favorecendo a interação entre compartimentos endossomais e o complexo de Golgi no fluxo retrógrado. A mutação em VPS13B traz conexões entre distúrbios do neurodesenvolvimento, tais como Síndrome de Cohen e TEA com comorbidades como a microcefalia e a deficiência intelectual. Por isso, células tronco pluripotentes induzidas (hiPSC) de paciente doador, podem fornecer interpretações funcionais do gene VPS13B e sua relação com o TEA, favorecendo o desenvolvimento de alternativas que propiciam qualidade de vida para indivíduos acometidos ainda sem perspectivas terapêuticas específicas. OBJETIVO: Investigar o tráfego intracelular de vesículas em hiPSCs derivadas de paciente autista com mutação missense no gene VPS13B. MATERIAL E MÉTODOS: As hiPSC foram obtidas de doador neurotiípico saudável (EA1) e de paciente com TEA (IM5). Duas variantes missense do gene VPS13B foram identificadas no exoma completo da paciente doadora com TEA, sendo N2968S confirmada por sequenciamento de Sanger. Para analisar os compartimentos, foram utilizadas técnicas de imunofluorescência, transfecção lentiviral, nuclaofecção plasmidial e microscopia eletrônica de transmissão (MET). RESULTADOS: As hiPSCs IM5 demonstraram alterações endocíticas, tais como aumento da atividade endossomal (RAB5 e RAB7), degradativa lisossomal (LAMP1) e autofágica (LC3) com redução no sistema de reciclagem (RAB11B). A linhagem IM5 também apresentou alterações ultraestruturais, revelando perda da morfologia normal do Complexo de Golgi e aumento da presença de vacúolos autofágicos. CONCLUSÕES: Nossos resultados sugerem que o gene VPS13B é relevante para o transporte intracelular em hiPSCs de modo que a perda parcial danificou a estrutura do complexo de Golgi, reduziu o sistema de reciclagem mediado por Rab11b, estimulando a degradação lisossomal e atividade autofágica.

INTRODUCTION: Autism spectrum disorder (ASD) is related to behavioral patterns, stereotypes, cognitive and social deficits. Endosomal and autophagic traffic ensure the dynamic flow of cytoplasmic loads and both pathways act in a complementary way, but through different mechanisms, preventing accumulation of damaged proteins and organelles. The Golgi complex is an intersection between several converging intracellular pathways where Rab proteins identify and direct the compartments along intracellular traffic. The VPS13B protein is associated with the membrane of the Golgi Complex in an interdependent manner in terms of functions and structure. The VPS13B protein transports phospholipids directly between endosomes, favoring the interaction between endosomal compartments and the Golgi apparatus in retrograde flow. The mutation in VPS13B brings connections between neurodevelopmental disorders such as Cohen Syndrome and ASD with comorbidities such as microcephaly and intellectual disability. Therefore, induced pluripotent stem cells (hiPSC) from a donor patient can provide functional interpretations of the VPS13B gene and its relationship with ASD, favoring the development of alternatives that provide quality of life for affected individuals without specific therapeutic perspectives. GOALS: To investigate the intracellular traffic of vesicles in hiPSCs derived from an autistic patient with a missense mutation in the VPS13B gene. MATERIAL AND METHODS: hiPSCs were obtained from a healthy neurotypical donor (EA1) and from an ASD patient (IM5). Two missense variants of the VPS13B gene were identified in the whole exome of the donor patient with ASD, N2968S being confirmed by Sanger sequencing. To analyze the compartments, immunofluorescence, lentiviral transfection, plasmid nucleofection and transmission electron microscopy (TEM) techniques were used. RESULTS: IM5 hiPSCs showed endocytic alterations, such as increased endosomal (RAB5 and RAB7), lysosomal degradative (LAMP1) and autophagic (LC3) activity with a reduction in the recycling system (RAB11B). The IM5 lineage also showed ultrastructural alterations, revealing loss of the normal morphology of the Golgi Complex and an increase in the presence of autophagic vacuoles. CONCLUSIONS: Our results suggest that the VPS13B gene is relevant for intracellular transport in hiPSCs so that the partial loss damaged the structure of the Golgi complex, reduced the Rab11b-mediated recycling system, stimulating lysosomal degradation and autophagic activity.

RESUMO A anemia falciforme (AF) é uma das formas de apresentação da doença falciforme (DF), que é caracterizada por eventos hematológicos como a hemólise e lesão endotelial que culminam na vaso-oclusão, levando ao aparecimento de diversas manifestações clínicas. A AF é resultado de uma alteração eritrocitária condicionada pela mutação de ponto no gene HBB que leva a formação da hemoglobina S (HbS). A hemoglobina fetal (HbF) é conhecida por afetar positivamente a clínica dos indivíduos com AF, por interferir no papel da HbS na alteração eritrocitária. Polimorfismos genéticos são conhecidos por modularem positivamente os fenótipos clínicos na AF, sendo o gene BCL11A um marcador importante nesse aspecto. Contudo, polimorfismos no gene BCL11A têm sido descritos por alterar negativamente o quadro clínico dos pacientes com AF. O presente estudo teve como objetivo associar biomarcadores laboratoriais na presença dos polimorfismos rs766432 (C>A) e rs6732518 (C>T) no gene BCL11A. Os marcadores hematológicos e bioquímicos foram investigados por métodos automatizados e os marcadores genéticos foram identificados pelas técnicas de reação em cadeia da polimerase e Restriction Fragment Length Polymorphism (PCR-RFLP). As análises estatísticas foram realizadas no software Graphpad prism na versão 9.0. A HbF apresentou associação estaticamente significante na presença do polimorfismo rs766432 (p=0,0306) bem como simultaneamente com o polimorfismo rs6732518 (p = 0,0302), e como as concentrações da HbS (p=0,0464). As concentrações de hemoglobina (Hb) (p=0,0008), hematócrito (Ht) (p=0,0243) (p=0,0329) e contagem de hemácias (Hm) (p=0,0069) (p=0,00466) estiveram elevadas na presença dos polimorfismos. As concentrações de bilirrubina total (p=0,0264) e direta (p=0,0039) apresentaram valores menores na presença do polimorfismo rs766432, bem como na co-herança dos polimorfismos (p=0,0073) (p=0,0154). Concentrações elevadas de colesterol HDL estiveram associados na presença do alelo variante do polimorfismo rs766432 (p=0,0402) e níveis elevados de alfa1- antitripsina estiveram associados na presença do alelo variante do polimorfismo rs6732518 (p=0,0320) e a ausência de correlação entre o a HbF e HDL(r=0,03705). Conclui-se que que polimorfismos no locus BCL11A são importantes para a variação dos níveis de HbF, e que podem apresentar efeito pleiotrópico pela determinação de parâmetros laboratoriais não-relacionados com os níveis de HbF.

ABSTRACT Sickle cell disease (SCD) is one of the types of sickle cell disease (SCD), which is characterized by hematological events such as hemolysis and endothelial damage that culminate in vaso-occlusion, leading to the appearance of several clinical manifestations. SCD is the result of erythrocyte alteration conditioned by a mutation in the HBB gene that leads to the formation of hemoglobin S (HbS). Fetal hemoglobin (HbF) is known to positively affect the clinical condition of individuals with SCA, by interfering with the role of HbS in erythrocyte alteration. Genetic polymorphisms are known to positively modulate clinical phenotypes of SCD, with the BCL11A gene being an important marker in this regard. However, polymorphisms in the BCL11A gene have been described as negatively change the clinical status of patients with SCD. The aim of this present study was associate laboratory biomarkers in the presence of polymorphisms rs766432 (C>A) and rs6732518 (C>T) in the BCL11A gene. Hematological and biochemical markers were investigated by automated methods and genetic markers were identified by polymerase chain reaction and Restriction Fragment Length Polymorphism (PCR-RFLP) techniques. Statistical analyses were performed using the Graphpad prism software, version 9.0. HbF showed a statistically significant association in the presence of the rs766432 polymorphism (p=0.0306) as well as simultaneously with the rs6732518 polymorphism (p = 0.0302), and HbS concentration (p=0.0464). The concentration of hemoglobin (Hb) (p=0.0008), hematocrit (Ht) (p=0.0243) (p=0.0329) and red blood cell count (Hm) (p=0.0069) (p= 0.00466) were high in the presence of polymorphisms. Total (p=0.0264) and direct (p=0.0039) bilirubin concentrations showed lower values in the presence of the rs766432 polymorphism, as well as in the co-inheritance of both polymorphisms (p=0.0073) (p=0, 0154). High concentrations of HDL cholesterol were associated with the presence of the variant allele of the polymorphism rs766432 (p=0.0402) and high levels of alpha1-antitrypsin were associated with the presence of the variant allele of the polymorphism rs6732518 (p=0.0320). There was no correlation between HbF and HDL (r=0.03705). It is concluded that polymorphisms in the BCL11A locus are important for the variation in HbF levels, and that they may have a pleiotropic effect by determining laboratory parameters not related to HbF levels.

As leishmanioses estão entre as enfermidades mais negligenciadas no mundo, afetando as populações mais pobres, principalmente em países em desenvolvimento. A leishmaniose cutânea (LC) é a forma mais frequente dessa doença, caracterizada por lesão(s) cutânea(s) ulceradas(s) com uma inflamação crônica descontrolada. Os mecanismos moleculares associados a regulações pós-transcricionais e a imunopatogênese da LC permanecem pouco compreendidos. Os microRNAs (miRNAs), pequenos RNAs endógenos não codificantes, surgiram nos últimos anos como moléculas-chave na regulação da expressão gênica. Dados do nosso grupo mostraram que o miR-193b, miR-671 e seus alvos estão fortemente correlacionados com o tempo de cura dos pacientes com LC. Estes achados sugerem que este eixo pode desempenhar um papel importante na LC, além de serem candidatos potenciais para o desenvolvimento de novas ferramentas para a doença. Além disso, comparamos o perfil de expressão de miRNAs com outras doenças inflamatórias de pele, onde apenas o miR-205 foi encontrado com a expressão significativamente inibida em todas as doenças. Diante dessas evidências, este estudo avaliou o papel funcional dos miR-193b e miR-205 na exposição in vitro de diferentes células humanas à Leishmania braziliensis (Lb). No primeiro capítulo, investigamos o perfil de expressão do miR-193b na infecção por Lb em macrófagos humanos in vitro. Inicialmente, análises de predição in silico sugeriram que esse miRNA tem o TREM1, TLR4, TNFRSF1B, CCR7, CD40 e STAT5 como alvos potenciais. Também foi possível validar a expressão desses alvos em novas biópsias de pacientes com LC e apenas a STAT5 não apresentou diferença estatística. Posteriormente, os ensaios com macrófagos infectados por Lb mostraram que a exposição por 30 minutos ou 4 horas reduz significativamente a expressão do miR-193b. Em contrapartida, a expressão do TREM1 foi significativamente aumentada nos dois tempos de infecção, diferente dos demais alvos que não apresentaram uma diferença estatística no perfil de expressão. Com o intuito de avaliar o papel funcional desse miRNA, macrófagos foram transfectados com miR-193b e infectados por Lb por 4 e 12 horas. Não foram observadas alterações nos potenciais alvos ou mediadores inflamatórios. No entanto, após 12 horas, houve redução na taxa de infecção e carga parasitária, sugerindo que este miRNA tem papel no controle da infecção. No segundo capítulo, validamos o perfil de expressão do miR205 em novas biópsias de pacientes com LC. Análises in silico mostraram os alvos do miR205, tanto na LC quanto nas demais doenças, e os resultados indicam que há uma especificidade dos alvos para cada doença avaliada. Sendo assim, validamos os dois genes alvo mais modulados na LC: SULF1 e o IRF1. Ambos, significativamente aumentados nas biópsias, confirmando os resultados das análises. Considerando que os queratinócitos desempenham um papel fundamental na regulação da inflamação da pele, avaliamos a expressão desse miRNA em queratinócitos humanos imortalizados (HaCats) expostos a Lb por 4 ou 24 horas, diante de lesões mecânicas. Os dados sugerem que, independente das lesões, a exposição das HaCats à Lb reduz a expressão desse miRNA. Nas HaCats transfectadas com o miRNA e expostas a Lb não foram observadas alterações na expressão dos alvos preditos, mediadores inflamatórios ou proliferação celular. No entanto, o miR-205 regulou a migração dessas células, uma vez que o percentual de fechamento de lesão no ensaio in vitro foi maior nos grupos transfectados com esse miRNA e menor com a exposição a Lb. Esses dados mostraram que a regulação negativa do miR-205 pode ser um fenômeno comum em doenças inflamatórias de pele, independente da sua etiologia, podendo atuar em processos básicos da biologia da pele e influenciando a cicatrização. Sendo assim, o miR-205 pode ser considerado um candidato promissor para aplicação na prática clínica. Em conjunto, essas análises demonstram o potencial desses dois miRNAs na LC, podendo atuar desde processos iniciais da inflamação até o processo de cicatrização da lesão.

Leishmaniasis is among the most neglected diseases in the world, affecting the poorest populations, mainly in developing countries. Cutaneous leishmaniasis (CL) is the most frequent form of this disease, characterized by ulcerated cutaneous lesion(s) with uncontrolled chronic inflammation. The molecular mechanisms associated with post-transcriptional regulations and CL immunopathogenesis remain poorly understood. MicroRNAs (miRNAs), small endogenous non-coding RNAs, have emerged in recent years as key molecules in the regulation of gene expression. Data from our group showed that miR-193b, miR-671 and their targets are strongly correlated with the healing time of patients with CL. These findings suggest that this axis may play an important role in CL, in addition to being potential candidates for the development of new tools for the disease. In addition, we compared the expression profile of miRNAs with other inflammatory skin diseases, where only miR-205 was found to have expression significantly inhibited in all diseases. Given this evidence, this study proposed to evaluate the functional role of miR-193b and miR-205 in the in vitro exposure of different human cells to Leishmania braziliensis (Lb). In the first chapter, we investigated the expression profile of miR193b in Lb infection in human macrophages in vitro. Initially, in silico prediction analyzes suggested that this miRNA has TREM1, TLR4, TNFRSF1B, CCR7, CD40 and STAT5 as potential targets. It was also possible to validate the expression of these targets in new biopsies from patients with CL and only STAT5 did not show a statistically significant difference. Subsequently, assays with Lb-infected macrophages showed that exposure for 30 minutes or 4 hours significantly reduced miR-193b expression. On the other hand, TREM-1 expression was significantly increased at the two times of infection, unlike the other targets that did not show a statistical difference in the expression profile. In order to assess the functional role of this miRNA, macrophages were transfected with miR-193b and infected with Lb for 4 and 12 hours. No changes in potential targets or inflammatory mediators were observed. However, after 12 hours, there was a reduction in the infection rate and parasite load, suggesting that this miRNA plays a role in infection control. In the second chapter, we validated the miR-205 expression profile in new biopsies from CL patients. In silico analyzes showed the targets of miR-205, both in CL and in other diseases, and the results indicate target specificity for each disease evaluated. Therefore, we validated the two most modulated target genes in CL: SULF1 and IRF1. Both were significantly increased in biopsies, confirming the results of the analyses. Considering that keratinocytes play a fundamental role in regulating skin inflammation, we evaluated the expression of this miRNA in human immortalized keratinocytes (HaCats) exposed to Lb for 4 or 24 hours, in the face of mechanical injuries. The data suggest that, regardless of the lesions, the exposure of HaCats to Lb reduces the expression of this miRNA. In HaCats transfected with miRNA and exposed to Lb, no alterations in the expression of predicted targets, inflammatory mediators or cell proliferation were observed. However, miR205 regulated the migration of these cells, since the percentage of wound closure in the in vitro assay was higher in groups transfected with this miRNA and smaller with exposure to Lb. These data showed that miR-205 downregulation may be a common phenomenon in inflammatory skin diseases, regardless of its etiology, and may act in basic processes of skin biology and influencing healing. Therefore, miR-205 can be considered a promising candidate for application in clinical practice. Together, these analyzes demonstrate the potential of these two miRNAs in CL, being able to act from the initial processes of inflammation to the wound healing process.

RESUMO A tuberculose (TB), causada pelo bacilo Mycobacterium tuberculosis, ainda é um problema de saúde pública mundial. A transmissão da TB acontece pelo ar, quando a pessoa infectada expele os bacilos de Koch através da expulsão de gotículas de Flügge, pelo meio da fala, tosse ou espirro, ficando suspenso no ar e podendo ser inalado por outras pessoas. Assim, a pessoa exposta pode-se tonar doente ou com a infecção latente (ILTB). Através do tempo, diversas pesquisas e iniciativas no mundo tem sido feitas para controlar a TB, entre eles a Estratégia para o Fim da TB da Organização Mundial da Saúde, cujo objetivo é eliminar a TB até o ano 2035. Apesar da implementação da estratégia, o cenário para o continente americano não tem melhorado muito em razão do incremento da incidência dos casos de TB, sendo o Brasil e o Peru os países com maior carga da doença. A fim de melhorar o panorama atual, são necessários conhecimentos atuais e adicionais da TB para a implementação de novas abordagens nos sistemas de saúde. Este trabalho de tese agrupa onze manuscritos que identificam potenciais determinantes clínicos e epidemiológicos da susceptibilidade à infecção pelo M. tuberculosis e da resposta terapêutica em pacientes TB. O primeiro estudo utiliza ferramentas estatísticas inovadoras para caracterizar a amostra coorte do RePORT-Brasil e a compara com a população nacional brasileira de TB, além disso identifica fatores associados aos desfechos desfavoráveis no tratamento anti-TB como o uso de drogas ilícitas, coinfecção por HIV e diabetes (DM) apenas na coorte do RePORT-Brasil. O rastreio de contactantes de casos TB e tratamento profilático daqueles com ILTB são importantes para o controle da transmissão da doença, em um dos nossos trabalhos identificamos que não completar a cascata de acompanhamento de ILTB estava associado independentemente com o aumento da idade, baixo status socioeconômico e a coinfecção por HIV. Além disso, seguindo com os resultados no primeiro estudo, demostramos mais uma vez que a DM está associada a um risco aumentado de desfechos desfavoráveis de tratamento, assim como da mortalidade em pacientes com TB pulmonar e comparamos esses resultados com os do SINAN-TB. Na coorte peruana achamos que a disglicemia persistente está associada também com desfechos desfavoráveis no tratamento. Adicionalmente, identificamos que a presença do HIV não afetou substancialmente a apresentação clínica em pessoas com TBDM. Adicionalmente, os contatos de pacientes com TB e pré-DM apresentaram maior risco de QuantiFERON positivo, no início do estudo e os contatos de pacientes que tinham TB-DM apresentaram um risco aumentado de ter uma conversão (QuantiFERON negativo no início para positivo no mês 6). Na triagem de DM nos casos de TB, identificamos diferenças quanto aos valores de HbA1c e FPG para diagnóstico da DM nas coortes do Peru (onde achamos alta prevalência de DM e de pré-DM em casos TB) e do Brasil. Finalmente, na coorte do Peru, a disglicemia (DM e pré-DM) afetou a apresentação lesões pulmonares nos casos de TB e padrões alimentares foram identificados no perfil de ingestão alimentar dos participantes desse estudo. Assim, os manuscritos que compõem a tese, adicionam informação relevante na identificação dos determinantes clínicos e epidemiológicos na infecção, transmissão e tratamento da TB, o que visa ser um suporte para a estratégias implementadas no sistema de saúde.

ABSTRACT Tuberculosis (TB), caused by Mycobacterium tuberculosis is still a worldwide public health problem. The transmission of TB occurs through the air, when the infected person releases the bacilli of Koch through the expulsion of droplets of Flügge, through coughing, sneezing or speaking, being suspended in the air and being inhaled by other people. Thus, the exposed person can become sick or with latent infection (ILTB). Over time, several research and initiatives in the world have been carried out to control TB, among them The Strategy for the End of TB of the World Health Organization, whose objective is to eliminate TB by the year 2035. Despite the implementation of the strategy, the scenario for the American region has not improved much due to the increase in the incidence of TB cases, with Brazil and Peru being the countries with the highest burden of the disease. Current knowledge of TB is needed to implement new approaches in health systems. This thesis work brings together eleven manuscripts that identify potential clinical and epidemiological determinants of susceptibility to M. tuberculosis infection and therapeutic response in TB patients. The first study uses innovative statistical tools to characterize the RePORT-Brasil cohort sample and compares it with the Brazilian national TB population, in addition to identifying factors associated with unfavorable outcomes in anti-TB treatment, such as the use of illicit drugs and co-infection with HIV and diabetes (DM) only in the RePORT-Brasil cohort. Screening contacts of TB cases and prophylactic treatment of those with ILTB are important for controlling disease transmission, we found that not completing the ILTB follow-up cascade was independently associated with increasing age, low socioeconomic status, and HIV co-infection. Furthermore, following the results of the first study, we demonstrated once again that DM is associated with an increased risk of unfavorable treatment outcomes, as well as mortality in patients with pulmonary TB, and we contrasted these results with those of SINAN. In the Peruvian court we find that persistent dysglycemia is also associated with unfavorable outcomes without treatment. Further, we also found that the presence of HIV did not substantially affect the clinical presentation in people with TBDM. Additionally, contacts of patients with TB and pre-DM were at risk of positive QuantiFERON at baseline and contacts of patients who had TBDM were at increased risk of having a conversion (QuantiFERON negative at baseline to positive at month 6). In screening for DM in TB cases, we identified differences regarding HbA1c and FPG values for the diagnosis of DM in the cohorts of Peru (where we found a high prevalence of DM and pre-DM in TB cases) and Brazil. Finally, in the Peru cohort, dysglycemia (DM and preDM) affected the presentation of lung lesions in TB cases and dietary patterns were identified in the food intake profile of the study participants. Thus, the manuscripts that make up the thesis add relevant information in identifying the clinical and epidemiological determinants of TB infection, transmission, and treatment, which aims to support strategies implemented in the health system.

RESUMO INTRODUÇÃO: As neoplasias mieloproliferativas (NMPs), mais especificamente a Policitemia vera (PV), a Trombocitemia essencial (TE) e a Mielofibrose primária (MFP), são um grupo de doenças clonais da medula óssea, compartilham a mutação JAK2V617F, entre outras características. As células estromais mesenquimais (MSCs) fazem parte do microambiente da medula óssea, e as influências mútuas das MSCs e do clone hematopoético são potencias determinante do fenótipo de NMP. OBJETIVO: O objetivo do presente trabalho foi investigar a expressão gênica e protéica das células estromais e hematopoéticas na PV, na TE e na MFP, em busca de moléculas que possam ser indicadas como biomarcadores de diagnóstico e prognóstico destas neoplasias. MATERIAL E MÉTODOS: As MSCs foram obtidas da medula óssea e os leucócitos do sangue periférico de pacientes com PV, TE, MFP e de indivíduos saudáveis. Extratos proteicos foram obtidos de MSCs de indivíduos saudáveis e de pacientes com PV e TE. O RNAs de MSCs e de leucócitos periféricos de controles saudáveis e de pacientes com PV, TE e MFP foi extraído pelo método de Trizol. Os RNAs foram convertidos em cDNA para as análises de qPCR e os extratos proteicos foram utilizados para análise por espectrometria de massas. RESULTADOS: A análise do proteoma de PV e TE comparados a controles indicou a maior expressão de VPS26A, CTTN, MAP4, TPD52L2, FAM175B, BAX em amostras de MSCs de PV comparadas a MSCs de indivíduos saudáveis, enquanto a proteína TNC mostrou-se menos expressa na PV. Nas MSCs de TE houve maior expressão de ALDH1A3, PON2 e SCP2; e menor expressão de RAB21 e RANBP1. As análises in silico identificaram genes diferencialmente expressos em MFP quando comparadas a controles, sendo que essas diferenças foram verificadas nas amostras de sangue periférico de um número maior de pacientes, com expressão diferencial significativa dos genes AVEN e CRACD, embora a expressão nos leucócitos tenha sido contrária àquela verificada nas MSCs de medula óssea. CONCLUSÕES: Os resultados obtidos indicam a expressão diferencial de algumas moléculas que podem ser potenciais marcadores das NMPs.

ABSTRACT INTRODUCTION: Myeloproliferative neoplasms (MPNs), more specifically Polycythemia vera (PV), Essential Thrombocythemia (ET) and Primary Myelofibrosis (PMF), are a group of clonal bone marrow diseases that share the JAK2V617F mutation. Mesenchymal stromal cells (MSCs) are part of the bone marrow microenvironment, and the mutual influences of MSCs and the hematopoietic clone are potential determinants of the MPN phenotype. AIM: The objective of the present work was to investigate the gene and protein expression of stromal and hematopoietic cells in PV, ET and PMF, in search of molecules that can be indicated as diagnostic and prognostic biomarkers. MATERIAL AND METHODS: MSCs were obtained from bone marrow and peripheral blood leukocytes from patients with PV, TE, PMF and healthy subjects. Four MSCs samples of each disease, excluding PMF, had their protein extracts isolated by cell lysis, and their RNAs extracted by the Trizol method. RNAs were converted into cDNA for qPCR analysis and protein extracts were used for mass spectrometry analysis. In silico analyses were performed using public transcriptome data in order to select differentially expressed genes in PMF. RESULTS: The proteome analysis of PV and TE compared to controls indicated a higher expression of VPS26A, CTTN, MAP4, TPD52L2, FAM175B, BAX in MSCs samples of PV compared to MSCs from healthy subjects, while the TNC protein was less expressed in the PV. In MSCs from TE, there was higher expression of ALDH1A3, PON2 and SCP2; and lower expression of RAB21 and RANBP1. In silico analyses identified genes differentially expressed in PMF when compared to controls, and these differences were verified in peripheral blood samples from a greater number of patients, with significant differential expression of the AVEN and CRACD genes, although the expression in leukocytes has been contrary to that seen in bone marrow MSCs. CONCLUSIONS: The results obtained indicate the differential expression of some molecules that can be potential markers of MPNs.

RESUMO Estima-se que aproximadamente um quarto da população global está infectada com Mycobacterium tuberculoses(Mtb). Polimorfismos genéticos do hospedeiro podem ser importantes na determinação da suscetibilidade à infecção Mtb, porém seu papel não é totalmente compreendido. No presente estudo, em uma coorte de contatos próximos de pacientes com TB pulmonar confirmados microbiologicamente atendidos em ambulatório de referência de TB no Rio de Janeiro, identificamos potenciais biomarcadores genéticos de suscetibilidade à infecção (conversão do teste tuberculínico (TST)) por Mtb e desenvolvimento de TB ativa, de forma prospectiva e retrospectiva. Além disso, realizamos uma revisão sistemática visando análise de biomarcadores genéticos adicionais. No primeiro estudo, diferente “single nucleotide polymorphims” (SNPs) foram estudados como fatores de risco para conversão do TST e desenvolvimento de TB: TLR2 (rs5743708), TLR4 (rs4986791), TNFA (rs361525), IFNG (rs2430561), IL1B (rs1143627). Em um segundo estudo, 7 SNPs adicionais foram testados para associação com positividade do TT: genes candidatos IFI16-PYHIN1-AIM2 (rs1101998, rs1633256, rs866484), IFIT5 (rs59633641, rs10887959), IFIT1 (rs304478, rs304498) e IRF7 (rs11246213). Ambos os estudos foram realizados em contatos de casos de TB pulmonar confirmados microbiologicamente em laboratórios de referência, no Rio de Janeiro. Finalmente, realizamos uma revisão sistemática para avaliar a associação entre todos os SNPs relatados de CD14 e NOD2 e a ocorrência de TB, e como essa associação pode diferir em populações étnicas distintas. No estudo prospectivo, entre os 526 participantes, 60 tiveram conversão no TST e 44 desenvolveram TB ativa durante o acompanhamento. Na análise de regressão multivariada observou-se que os SNPs em genes TLR4 (odds ratio [OR]: 62,8, intervalo de confiança de 95% [IC 95%]: 7,5–525,3) e TNFA (OR: 4,2, IC 95%: 1,9–9,5) foram independentemente associados à conversão no TST. No estudo retrospectivo foram examinados 482 contatos, dos quais 296 contatos apresentaram TST positivo. Em um modelo multivariável, observamos que no modelo recessivo o SNP PYHIN1-IFI16-AIM2 rs1101998 (OR ajustado [aOR] = 2,90; IC 95% = 1,24-6,78; p = 0,014) e rs1633256 (aOR = 10,1; IC 95% = 2,20- 46,28; p = 0,003) foram associados a um risco aumentado de reatividade no TST. Na revisão sistemática, foram incluídos, 13 estudos que preencheram os critérios de seleção. Destes, 9 foram investigados do gene CD14 e em 6 foi relatada uma associação significativa entre o alelo T e os genótipos TT do SNP rs2569190 e aumento do risco de TB. Ademais, em 4 estudos foram relatadas relações entre os SNPs do gene NOD2 e a TB, e destes, foram observadas associações significativas de rs1861759 e rs7194886 com maior risco de TB em uma população chinesa Han em 2 estudos. Os resultados sugerem associações entre polimorfismos dos genes da imunidade e as probabilidades de infecção e/ou adoecimento por Mtb. No intuito de promover conhecimento sobre fatores imunogenétcos em TB na presente análise, foram apresentadas associações entre polimorfismos de genes relacionados à imunidade e as probabilidades de infecção e/ou adoecimento por Mtb.

ABSTRACT Mycobacterium tuberculosis (Mtb) infection affects approximately a quarter of the global population. Host genetic polymorphisms may be important in determining susceptibility to Mtb infection, but their role is not fully understood. In a first study, different SNPs were tested as risk factors for tuberculin skin test (TST)conversion and development of Tuberculosis (TB): TLR2 (rs5743708), TLR4 (rs4986791), TNFA (rs361525), IFNG (rs2430561), IL1B (rs1143627). In a second study, seven additional SNPs were tested for association with TT positivity: candidate genes IFI16-PYHIN1-AIM2 (rs1101998, rs1633256, rs866484), IFIT5 (rs59633641, rs10887959), IFIT1 (rs304478, rs730449.8), and IRF7 (rs11246213). Both studies were conducted on contacts of microbiologically confirmed pulmonary TB cases in reference laboratories. Finally, we performed a systematic review to assess the association between CD14 and NOD2 reported polymorphisms and Mtb diseases, and how this association might differ in distinct ethnic populations. In the prospective study, among the 526 participants, 60 had a conversion to TT, and 44 developed active TB during follow-up. Multivariate regression analysis demonstrated that SNPs in TLR4 genes (odds ratio [OR]: 62, 8, 95% confidence interval [95% CI: 7.5–525.3) and TNFA (OR: 4.2, 95% CI: 1.9– 9.5) were independently associated with TT conversion. In the retrospective studio outside 482 contacts were examined, of which 296 contacts had positive TT. In a multivariate model, we observed in the recessive model that PYHIN1-IFI16-AIM2 rs1101998 (adjusted OR [aOR] = 2.90; 95% CI = 1.24-6.78; p = 0.014) and rs1633256 (aOR = 10, 1; 95% CI = 2.20-46.28; p = 0.003) were associated with an increased risk of TT positivity. In the systematic review, thirteen studies met the selection criteria. Of these, nine investigated CD14 SNPs and six reported a significant association between the T allele and the TT genotypes of SNP rs2569190 and increased risk of Mtb disease. In addition, four studies reported data finding the relationship between NOD2 SNPs and Mtb disease risk, with two reporting significant associations of rs1861759 and rs7194886 and increased risk of Mtb disease in a Han Chinese population. The results suggest associations between immunityrelated genes polymorphisms and the probabilities of Mtb infection. RESUMO INTRODUÇÃO: A Leishmaniose Tegumentar (LT) é uma doença parasitária que pode resultar em um amplo espectro de formas clínicas a depender de determinantes do hospedeiro e do parasito. Entender essas características nos diferentes desfechos clínicos é importante a fim de identificar novos alvos terapêuticos. Fármacos antimoniais pentavalentes, são a primeira classe de drogas utilizadas durante o tratamento das leishmanioses. No entanto, cerca de 45% dos pacientes apresentam falha na terapia no Brasil. Nesse sentido, a identificação de potenciais biomarcadores de gravidade de doença ou controle do parasito pode favorecer o desenvolvimento de terapias direcionadas ao hospedeiro que podem incrementar o manejo clínico de casos graves/complicados. OBJETIVO: Neste estudo, avaliamos biomarcadores de gravidade de doença e falha terapêutica em pacientes com leishmaniose tegumentar e o papel das resolvinas na resistência à infecção por Leishmania. RESULTADOS: Nossos resultados demonstram que pacientes com LCD apresentam uma ativação diferencial da via das poliaminas e aminoácidos quando comparados a LCL ou LCM podendo ser utilizada como biomarcador de gravidade de doença. Além disso, encontramos uma bioassinatura influenciada por proteínas plasmáticas e mediadores lipídicos que prediz com alta acurácia a falha terapêutica na LT. Experimentos adicionais in vitro utilizando neutrófilos humanos revelaram que a RvD1 promove a replicação intracelular da L. braziliensis, no entanto, o mecanismo por trás desse efeito ainda precisa ser investigado. CONCLUSÃO: Os resultados sugerem que as vias de produção das poliaminas e mediadores lipídicos podem ser utilizados como biomarcadores de gravidade de doença e falha terapêutica e que a RvD1 favorece a resistência do parasito, podendo em conjunto servir como uma potencial estratégia terapêutica.

ABSTRACT INTRODUCTION: Tegumentary leishmaniasis (TL) is a parasitic disease that can result in wide spectrum clinical manifestations depending on host and parasite determinants. Understanding these characteristics in different clinical outcomes is important to identify novel therapeutic targets. Pentavalent antimonials are the first-line drugs used to treat Leishmaniosis. However, occurrence of treatment failure in Brazil can be as high as 45%. In this context, identification of potential biomarkers of disease severity or parasite control may favor the development of host-oriented therapies that may increase the clinical management of severe/complicated cases. AIM: In this study, we evaluated biomarkers of disease severity and therapeutic failure in patients with tegumentary leishmaniasis and the role of resolving in the resistance to Leishmania infection. RESULTS: Our results show that patients with DCL present a differential activation of the polyamine and amino acid pathway when compared to LCL and MCL and this pathway can be used as a biomarker of disease severity. In addition, we found a biosignature influenced by plasma proteins and lipid mediators that accurately predicted treatment failure in LT. Additional in vitro experiments using human neutrophils revealed that RvD1 promotes intracellular replication of L. braziliensis however, the mechanism behind this effect still needs to be investigated. CONCLUSION: The results suggest that the production pathways of polyamines and lipid mediators can be used as biomarkers of disease severity and therapeutic failure and that RvD1 favors parasite resistance, and can together serve as a potential therapeutic strategy

RESUMO INTRODUÇÃO: O diagnóstico da tuberculose (TB) é realizado através da detecção do M. tuberculosis (MTB) em testes microbiológicos, como baciloscopia e cultura e ensaios de biologia molecular. A intepretação desses exames é limitada, pois não trazem informações sobre a repercussão sistêmica, celular, inflamatória e apresentam baixa sensibilidade em indivíduos paucibacilar. OBJETIVO: Avaliar a ativação celular e as alterações fenotípicas em linfócitos T CD4+ em indivíduos com tuberculose, na presença ou não do HIV. MÉTODOS: Inicialmente, em uma coorte do Brasil, validamos a utilização de marcadores de ativação celular CD38, HLADR e/ou Ki67 em células TCD4+ específicas para MTB em discriminar tuberculose Pulmonar (TBP), Tuberculose extrapulmonar (TBEP) e tuberculose latente (ILTB). Testamos ainda o efeito da coinfecção pelo HIV no desempenho do diagnóstico. Juntou-se a esta tese, outro manuscrito abrangendo pacientes do sul da Índia com TB-HIV antes e após o início da terapia antiretroviral (TARV), determinando as alterações fenotípicas nas células T durante a síndrome inflamatória de reconstituição imune (SIRI) e sua relação com a inflamação sistêmica. Neste estudo, analisamos subpopulações de linfócitos T CD4 e biomarcadores no sangue periférico. RESULTADOS: Os pacientes com TBEP e TBP apresentaram uma maior frequência de células T CD4+ IFN-γ+ expressando CD38, HLADR em comparação com ILTB. Além disso, as frequências de células HLADR+ ou Ki67+ distinguiram com precisão TBEP do TBP. A infecção pelo HIV não afetou a capacidade desses marcadores de distinguir tuberculose ativa de ILTB ou TBEP de TBP. No segundo estudo, a frequência de células T CD4 naive (CD27+CD45RO−), bem como de memória efetora (CD27−CD45RO+) distinguiram os indivíduos que desenvolveram a síndrome inflamatória de reconstituição dos que não apresentaram, na 2°-6° semana após o início da TARV. Análises posteriores revelaram que o TARV reconstituiu diferentes quantidades de subconjuntos de linfócitos T CD4+ com expansão preferencial de células CXCR3+ CCR6− em pacientes com TB-SIRI. Além disso, houve uma expansão e restauração funcional de linfócitos T CD4+ CXCR3−CCR6+ expressando marcadores de memória central (CD27+CD45RO+) e citocinas correspondentes, com redução nas células CXCR3−CCR6+ após TARV apenas naqueles que desenvolveram TB-SIRI. CONCLUSÃO: Os marcadores de ativação celular em células T CD4+ específicas para MTB distinguiram TB ativa de ILTB e TBEP da TBP, independentemente do status de infecção pelo HIV. Os subconjuntos de células T CD4+ estão fortemente associadas com SIRI.

ABSTRACT INTRODUCTION: The diagnosis of tuberculosis (TB) is performed by means of the detection of M. tuberculosis (MTB) in microbiological tests, such as smear and culture and molecular biology assays. The interpretation of these tests is limited, as they do not provide information on the systemic, cellular, and inflammatory repercussions and they have low sensitivity in paucibacillary individuals. OBJECTIVE: To evaluate cell activation and phenotypic changes in CD4+ T lymphocytes in individuals with tuberculosis, with or without HIV. METHODS: Initially, in a cohort from Brazil, the use of CD38, HLADR and/or Ki67 cell activation markers on MTB-specific CD4+ T cells to discriminate pulmonary tuberculosis (PTB) from extrapulmonary tuberculosis (EPTB) from latent tuberculosis infection (LTBI), as well as EPTB from PTB, was validated. We also tested the effect of HIV coinfection on diagnostic performance. Added to this thesis, another manuscript covering South Indian patients with TB-HIV before and after initiation of ART, determining the phenotypic changes in CD4+ T cells during Tuberculosis-immune reconstitution inflammatory (SIRI) and their relationship with systemic inflammation. In this study, we analyzed subpopulations of T lymphocytes and biomarkers in peripheral blood. RESULTS: EPTB and PTB patients had a higher frequency of T CD4+ IFN-γ+ cells expressing CD38, HLADR compared to LTBI. Furthermore, the frequencies of HLADR+ or Ki67+ cells accurately distinguished EPTB from PTB. HIV infection did not affect the ability of these markers to distinguish active tuberculosis from LTBI or EPTB from PTB. In the second study, the frequency of naive CD4 T cells (CD27+CD45RO−), as well as of effector memory (CD27−CD45RO+) distinguished individuals who developed the inflammatory reconstitution syndrome from those who did not, in the 2nd-6th week after starting ART. Further analysis revealed that ART reconstituted different amounts of subsets of CD4+ T lymphocytes with preferential expansion of CXCR3+ CCR6− cells in TB-SIRI patients. In addition, there was an expansion and functional restoration of CXCR3−CCR6 CD4+ lymphocytes expressing central memory markers (CD27+ CD45RO+) and corresponding cytokines, with a reduction in CXCR3−CCR6+ cells after ART only in those who developed TB-SIRI. CONCLUSION: Cell activation markers on CD4+ T cells specific for MTB distinguished active TB from LTBI and EPTB from PTB, regardless of HIV infection status. CD4+ T cell subsets are strongly associated with SIRI.

RESUMO A leishmaniose cutânea (LC) é um grave problema de saúde pública. Por falta de interesse comercial, o atual arsenal terapêutico está desatualizado e apresenta sérias limitações. O presente estudo empregou ferramentas nanotecnológicas e explorou a biodiversidade da flora brasileira para avaliar quatro possíveis candidatos à terapia da LC: Carreadores Lipídicos Nanoestruturados contendo Anfotericina B (CLN-AnB), extrato de própolis verde brasileiro padronizado (EPP-AF®) e as lignanas iangambina e epi-iangambina em modelos in vitro e/ou in vivo de infecção por L. amazonensis ou L. braziliensis, ambos agentes etiológicos da LC no Brasil. Os ensaios in vitro revelaram a rápida absorção de CLN-AnB pelos macrófagos após 1h de tratamento, contribuindo para redução da citotoxicidade e efeito leishmanicida em baixas concentrações, não associado à produção de TNF-𝛼 ou óxido nítrico. Eficácia semelhante foi observada entre os tratamentos CLN-AnB e antimoniato de meglumina em relação à redução de lesões e redução da carga parasitária em camundongos BALB/c infectados com L. braziliensis. Por outro lado, macrófagos infectados por L. amazonensis e tratados com EPP-AF® apresentaram modulação na produção de mediadores pró e anti-inflamatórios, além da ativação da via de sinalização ERK 1/2, enquanto apresentou reduzido índice de infecção. Ademais, o tratamento tópico com gel EPP-AF® resultou na redução do tamanho da lesão em camundongos BALB/c infectados com L. amazonensis, sem possuir efeito na redução da carga parasitária ou na produção de mediador inflamatório sistêmico, bem como nos linfonodos ou no local de infecção (derme da orelha). Finalmente, demonstramos pela primeira vez os efeitos leishmanicida e imunomodulador da iangambina e da epi-iangambina em macrófagos infectados por L. amazonensis e L. braziliensis. Em suma, este projeto possui potencial inovador para identificar novos candidatos à terapia de CL, além de aprofundar a busca por compostos bioativos no contexto da biodiversidade brasileira.

ABSTRACT Cutaneous leishmaniasis (CL) is a serious public health concern. Due to lacking commercial interest, the current therapeutic arsenal is outdated and presents serious limitations. The present study employed nanotechnological tools and explored the biodiversity of Brazilian flora to evaluate four possible candidates for CL therapy: Nanostructured Lipid Carriers containing Amphotericin B (CLN-AnB), Standardized Brazilian green propolis extract (EPP-AF®) and the yangambin and epi-yangambin lignans in in vitro and/or in vivo models of L. amazonensis or L. braziliensis infection, both etiological agents of CL in Brazil. In vitro assays revealed the rapid uptake of CLN-AnB by macrophages after 1h of treatment, contributing to reduced cytotoxicity and a leishmanicidal effect at low concentrations, in the absence of TNF-𝛼 or nitric oxide production. Similar efficacy was observed between the CLN-AnB and meglumine antimoniate treatments with respect to lesion reduction and decreased parasite burden in L. braziliensis-infected BALB/c mice. On the other hand, L. amazonensis-infected macrophages treated with EPP-AF® modulated the production of pro- and anti-inflammatory mediators, in addition to activating the ERK 1/2 signaling pathway, while also reducing infection index. Furthermore, topical treatment with EPP-AF® gel resulted in reduced lesion size in L. amazonensis-infected BALB/c mice in the absence of reduced parasite load or altered systemic inflammatory mediator production, as well as in the lymph nodes or at the site of infection (ear dermis). Finally, the leishmanicidal and immunomodulatory effects of yangambin and epi-yangambin were demonstrated for the first time in both L. amazonensis- and L. braziliensis-infected macrophages. In sum, this project possesses the innovative potential to identify novel candidates for CL therapy, in addition to furthering the search for bioactive compounds in the context of Brazilian biodiversity.

RESUMO INTRODUÇÃO: Com milhões de casos e mortes confirmadas em todo o mundo, a pandemia da doença do coronavírus 2019 (COVID-19), causada pela infecção do novo coronavírus, denominada síndrome respiratória aguda grave coronavírus 2 (SARS-CoV-2), se tornou uma preocupação mundial. Sabe-se que níveis elevados de glicose na corrente sanguínea são um dos principais fatores de risco para o agravamento da doença. No entanto, ainda não estão elucidados os reais mecanismos envolvidos no agravamento da COVID-19 em indivíduos com pré-diabetes e diabetes. OBJETIVO: O objetivo do presente estudo foi avaliar o papel do diabetes e do pré-diabetes no agravamento da COVID-19. MATERIAL E MÉTODOS: Inicialmente, foram identificados possíveis genes relacionados a gravidade da COVID-19 em indivíduos com diabetes a partir de dados públicos de expressão gênica em células mononucleares do sangue periférico (PBMCs). Em seguida, foram validados alvos de importância para a infecção do SARS-CoV-2 em PBMCs de pacientes com e sem diabetes. Dosagem de mediadores inflamatórios sistêmicos e análises de correlação foram utilizados para identificar fatores associados à gravidade da COVID-19 em pacientes com a glicemia alterada. RESULTADOS: As análises de dados globais públicos revelaram a via do metabolismo do leucotrieno como uma via potencialmente associada a condições respiratórias em pacientes com diabetes. Em seguida, identificamos, na fase aguda da COVID-19, um aumento na expressão dos genes ALOX5 e ACE2/TMPRSS2 em PBMCs de indivíduos com diabetes. Nesses pacientes, também foi encontrado um aumento dos níveis séricos de LTB4 e IL-6 em comparação a pacientes sem diabetes. O aumento de IL-6 observado em indivíduos com diabetes estava associado com maior internação em unidade de terapia intensiva. Em conjunto, os dados mostram que o diabetes, com a participação da via do LTB4, pode levar a COVID-19 grave, induzindo lesão pulmonar mais intensa e maior tempo de doença. Curiosamente, pacientes com pré-diabetes, sob a participação da produção de IL-6, também evoluem para a COVID-19 grave em maior frequência, considerando a redução nas taxas de troca gasosa e maior tempo de hospitalização. Contudo, o pré-diabetes não induziu sequelas da COVID-19 distintas daquelas de indivíduos sem diabetes. Além disso, nós mostramos também, que exames laboratoriais de rotina podem ser utilizados para identificar altos/baixos produtores de IL-6, cujos níveis estão relacionados com a gravidade da COVID-19. CONCLUSÃO: O aumento da produção de LTB4 e IL-6 observado em indivíduos com diabetes e pré-diabetes, respectivamente, pode piorar o desfecho da COVID-19.

ABSTRACT INTRODUCTION: With millions of confirmed cases and deaths worldwide, the coronavirus disease 2019 (COVID-19) pandemic caused by the infection of the novel coronavirus called severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) has become a worldwide concern. It is known that high levels of glucose in the bloodstream is one of the main risk factors for the worsening of the disease. However, the real mechanisms involved in the aggravation of COVID-19 in individuals with prediabetes and diabetes are not yet elucidated. OBJECTIVE: Thus, the aim of the present study was to evaluate the influence of diabetes and pre-diabetes on the worsening of COVID-19. MATERIAL AND METHODS: Initially, possible genes related to the severity of COVID-19 in individuals with diabetes were identified from public gene expression data in peripheral blood mononuclear cells (PBMCs). Then, we validated important targets for SARS-CoV-2 infection in PBMCs of patients with and without diabetes. Systemic inflammatory mediators and correlation analyzes were performed to identify factors associated with COVID-19 severity in patients with altered blood glucose. RESULTS: Public data analyzes revealed the leukotriene metabolism as a pathway potentially associated with respiratory conditions in patients with diabetes. Thus, we identified, during the acute phase of COVID-19, an increase in the expression of ALOX5 and ACE2/TMPRSS2 in PBMCs of individuals with diabetes. In these patients, we also found an increase in serum levels of LTB4 and IL-6 compared to patients without diabetes. The increase in IL-6 observed in individuals with diabetes was associated with longer intensive care unit admission. Taken together, the data show that diabetes, with the participation of the LTB4 pathway, induces a more severe disease, with more intense lung damage (gas exchange rates) and longer disease duration. Interestingly, patients with prediabetes, under the participation of IL-6 production, also develop to severe COVID-19 more frequently, considering the reduction in gas exchange rates and longer hospitalization time. However, prediabetes did not induce sequelae of COVID-19 distinct from those of subjects without diabetes. In addition, we also show that routine laboratory tests can be used to identify different producers of IL-6, which is related to the severity of COVID-19. CONCLUSION: The increased production of LTB4 and IL-6 seen in individuals with diabetes and prediabetes, respectively, may worsen the outcome of COVID-19.

INTRODUÇÃO: a doença falciforme (DF) é uma doença genética, caracterizada pela presença da hemoglobina variante S (HbS) associada a outra hemoglobina variante ou talassemia. A etiologia da DF decorre da mutação de ponto no sexto códon do gene da globina β, com substituição do ácido glutâmico pela valina na cadeia polipeptídica. A forma mais grave da DF é a anemia falciforme (AF), caracterizada pela presença em homozigose do aleno beta S, dando origem ao genótipo HbSS. As complicações da DF variam em gravidade e podem ser classificadas como agudas ou crônicas. A etiologia das hemoglobinas variantes HbS e HbC é de origem africana e está associada ao período da invasão europeia em território brasileiro, o que pode explicar o motivo de a DF apresentar prevalência elevada em indivíduos afrodescendentes. O presente estudo tem como objetivo estudo tem como objetivo principal investigar indicadores de morbidade, de mortalidade e de prognóstico associados a DF no estado da Bahia. MÉTODOS: foram realizados estudos envolvendo 178 indivíduos com DF entre 6 e 54 anos de idade em tratamento em um centro de referência na cidade de Salvador, Bahia. O estudo abrangeu o período de 2006 a 2017. As informações referentes às manifestações clínicas, o uso de medicamentos, os dados laboratorias, os dados socioeconômicos e sociodemográficos foram obtidas através da consulta aos prontuários clínicos dos pacientes estudados. RESULTADOS: o primeiro manuscrito teve a casuística de 177 indivíduos com DF, sendo 129 com o genótipo HbSS e 48 com o genótipo HbSC. Esse estudo teve como objetivo descrever a frequência da população com DF nos territórios de identidade (TI) do estado da Bahia. Os resultados mostraram que os indivíduos eram oriundos de 23 TI. A maior prevalência da DF foi encontrada na TI da Região Metropolitana de Salvador (RMS), seguida pelas TIs do Recôncavo da Bahia e Portal do Sertão. Esses dados reforçam a presença histórica de africanos escravizados na Bahia. O segundo manuscrito teve a casuística de 178 indivíduos com DF analisados em um estudo de seguimento retrospectivo entre os anos de 2006 e 2017. O objetivo desse manuscrito foi descrever as principais manifestações clínicas presentes na população estudada. Observamos que o evento vaso-oclusivo/ crise de dor foram as manifestações clínicas mais frequentes e o infarto medular, a menos frequente. Assim, pudemos caracterizar a população de DF atendida nesse centro de referência. O terceiro manuscrito foi resultado de uma coorte retrospectiva envolvendo a casuística de 99 pacientes com DF em tratamento com o medicamanto hidroxiureia (HU). Nesse estudo, analisamos as manifestações clínicas e laboratoriais dos pacientes com DF antes e durante o uso de HU e percebemos que a HU foi capaz de reduzir as manifestações clínicas da população analisada, bem como melhorar seus parâmetros laboratorias. CONCLUSÕES: a partir dos resultados obtidos, observou-se que as manifestações clínicas que atingem indivíduos com DF continuam contribuindo para a prevalência elevada de morbidade e mortalidade, principalmente na população mais vulnerável economicamente. Acrescenta-se ainda que os dados apresentados nesse trabalho mostram a presença marcante dos descendentes de africanos escravizados no Brasil. Nossos dados também confirmam a persistência das desigualdades sociais e econômicas que afetam esses indivíduos ao longo do desenvolvimento do Brasil, bem como esclarece que o tratamento farmacológico de pacientes HbSS e HbSC com HU continua apresentando benefícios clínicos e laboratoriais significativos a longo prazo.

INTRODUCTION: sickle cell disease (SCD) is a genetic disease characterized by the presence of variant hemoglobin S (HbS) associated with another variant hemoglobin or thalassemia. The etiology of SCD stems from a point mutation in the sixth codon of the β-globin gene, with substitution of glutamic acid for valine in the polypeptide chain. The most severe form of SCD is sickle cell anemia (SCA), characterized by the homozygous presence of the beta S allen, giving rise to the HbSS genotype. SCD complications vary in severity and can be classified as acute or chronic. The etiology of the hemoglobin variants HbS and HbC is of African origin and is associated with the period of European invasion of Brazilian territory, which may explain why SCD has a high prevalence in Afro-descendant individuals. The present study aims to investigate indicators of morbidity, mortality and prognosis associated with SCD in the state of Bahia. METHODS: studies were carried out involving 178 individuals with SCD between 6 and 54 years of age undergoing treatment at a referral center in the city of Salvador, Bahia. The study covered the period from 2006 to 2017. Information regarding clinical manifestations, medication use, laboratory data, socioeconomic and sociodemographic data were obtained by consulting the clinical records of the patients studied. RESULTS: the first manuscript had a sample of 177 individuals with SCD, 129 with the HbSS genotype and 48 with the HbSC genotype. This study aimed to describe the frequency of the population with SCD in the identity territories (IT) of the state of Bahia. The results showed that the individuals came from 23 TIs. The highest prevalence of SCD was found in the IL in the Metropolitan Region of Salvador (RMS), followed by the ILs in the Recôncavo da Bahia and Portal do Sertão. These data reinforce the historical presence of enslaved Africans in Bahia. The second manuscript had a sample of 178 individuals with SCD analyzed in a retrospective follow-up study between 2006 and 2017. The objective of this manuscript was to describe the main clinical manifestations present in the population studied. We observed that the vaso-occlusive event/pain crisis were the most frequent clinical manifestations and the spinal cord infarction, the least frequent. Thus, we were able to characterize the SCD population served at this referral center. The third manuscript was the result of a retrospective cohort involving a series of 99 patients with SCD undergoing treatment with the drug hydroxyurea (HU). In this study, we analyzed the clinical and laboratory manifestations of patients with SCD before and during the use of HU and we realized that HU was able to reduce the clinical manifestations of the analyzed population, as well as improve their laboratory parameters. CONCLUSIONS: from the results obtained, it was observed that the clinical manifestations that affect individuals with SCD continue to contribute to the high prevalence of morbidity and mortality, especially in the most economically vulnerable population. It is also added that the data presented in this work show the marked presence of descendants of enslaved Africans in Brazil. Our data also confirm the persistence of social and economic inequalities that affect these individuals throughout Brazil's development, as well as clarify that the pharmacological treatment of HbSS and HbSC patients with HU continues to have significant clinical and laboratory benefits in the long term.

A tuberculose (TB) é uma doença infectocontagiosa de difícil controle e os mecanismos envolvidos na progressão da doença não são totalmente estabelecidos. O perfil de expressão gênica vem sendo estudado com o objetivo de validar potenciais biomarcadores que auxiliem no entendimento dos processos envolvidos na TB e no desenvolvimento de estratégias para o combate da doença. A vacinação com a BCG vem sendo apontada com a capacidade de modular a resposta do hospedeiro e sua eficácia contra a TB têm sido amplamente discutida, porém ainda não se sabe totalmente sobre quais seriam esses efeitos e a sua duração no organismo. Por meio de uma metanálise de dados de transcriptoma utilizando o repositório GEO, conseguimos demonstrar que a vacinação com a BCG é capaz de gerar efeitos duradouros no hospedeiro, afetando a modulação de genes específicos. Ao comparar os grupos de não infectados, infectados e doentes que não foram vacinados, observamos 100 genes (92 modulados negativamente e 8 modulados positivamente), associados principalmente com vias e funções ligadas ao metabolismo. Já entre os indivíduos vacinados, comparando-se os mesmos grupos, foram observados 53 genes, sendo 18 superexpressos e 35 subexpressos, associados principalmente aos processos de imunidade. Em uma metanálise secundária, verificamos também assinaturas distintas ligadas à vacina, com 5 genes subexpressos identificados para os vacinados e 2 genes (1 super e 1 subexpresso) para os não vacinados. Nossos resultados ainda apontaram que um gene da subfamília Rab11 (composta por outros dois membros de elevada homologia, o RAB11A e RAB11B), o RAB25, apresentou-se superexpresso em indivíduos doentes e não vacinados com a BCG, quando comparados também com os doentes, porém vacinados. Identificamos durante as nossas avaliações o RAB11A como o gene-alvo do nosso estudo. Em amostras de sangue total de uma coorte de pacientes com tuberculose ativa da nossa população, observamos valores médios de expressão maiores para o gene RAB11A em indivíduos doentes e não vacinados, quando comparados aos indivíduos vacinados, corroborando com os dados da metanálise. Vimos também que os valores de expressão deste gene são menores em células THP-1 infectadas com a cepa vacinal BCG em comparação com as não infectadas. Para investigação futura do papel deste gene, realizamos com sucesso o silenciamento gênico em células THP-1 através da metodologia de RNA de interferência. Com este trabalho, conseguimos então identificar as assinaturas transcricionais associadas à vacinação com a BCG e apontar um alvo que merece ser melhor estudado na tuberculose.

Tuberculosis (TB) is a chronic infectious disease difficult to manage and the mechanisms involved in disease progression are not fully established. The gene expression profile has been studied with the objective of validating potential biomarkers that help in the understanding of the processes involved in TB and in the development of strategies against disease. Vaccination with BCG has been pointed out with the possibility of modulating the host response and its efficacy against TB has been widely discussed, but it is still not fully known about what these effects would be and their duration in the organism. Through a meta-analysis of transcriptome data using GEO repository, we were able to demonstrate that vaccination with BCG is capable of generating lasting effects in the host affecting the modulation of specific genes. When comparing the groups of uninfected, infected and sick that were unvaccinated, 100 genes were observed (92 down-regulated and 8 up-regulated), mainly associated with pathways and functions linked to metabolism. Among vaccinated individuals, comparing the same groups, 53 genes were observed, being 18 overexpressed and 35 underexpressed, mainly linked to immunity processes. In a secondary meta-analysis, we also found distinct vaccine-linked signatures, with 5 underexpressed genes identified for the vaccinated and 2 genes (1 over and 1 underexpressed) for the unvaccinated. Our results also showed that a gene of the Rab11 subfamily (composed of two other members of high homology, RAB11A and RAB11B), RAB25, was overexpressed in sick individuals unvaccinated with BCG, when also compared sick individuals, but vaccinated. During our evaluations, we identified RAB11A as the target of our study. In whole blood samples from a cohort of patients with active tuberculosis from our population, we observed higher mean expression values for the RAB11A gene in sick and unvaccinated individuals compared to vaccinated, corroborating the meta-analysis data. We also saw that the expression values of this gene are lower in THP-1 cells infected with BCG strain compared to uninfected. For future investigation of the role of this gene, we successfully performed gene silencing in THP-1 cells using RNA interference methodology. With this work, we were able to identify the transcriptional signatures associated with BCG vaccination and point out a target that deserves to be better studied in tuberculosis.

RESUMO Corpúsculos lipídicos (CLs) são organelas citoplasmáticas responsáveis pelo armazenamento de lipídios neutros e pelo metabolismo de mediadores em muitos tipos de células. Dados recentes mostram a existência de CLs e produção de eicosanoides em Leishmania. Contudo, estudos sobre a formação e modulação de CLs, bem como, da síntese de lipídios bioativos em diferentes espécies de Leishmania são escassos. Assim, primeiramente foram avaliadas por microscopia ótica a formação de CLs em L. infantum, L. amazonensis e L. braziliensis, bem como, a modulação destas organelas estimuladas com os ácidos eicosapentaenóico (EPA), docosahexaenoico (DHA) e araquidônico (AA). Os mediadores produzidos por estas espécies foram quantificados por cromatografia líquida acoplada à espectrometria de massas (HPLC-MS) e a expressão da glicoproteína de 63 kDa (GP63) e da prostaglandina F2α sintase (PGFS) foram avaliadas por imunoblotting e ELISA. Por fim, as sequencias genômicas e os resíduos do sítio ativo dessas proteínas foram analisadas em espécies de Leishmania do Velho e Novo Mundo. Observamos maior número de CLs durante metaciclogênese e após estimulação com EPA, DHA e AA, em L. infantum e L. braziliensis. Além disso, identificamos 12 lipídios bioativos, estimulados principalmente por AA. A partir das análises in silico, notamos que as sequências de GP63 e PGFS são mais similares entre as espécies de mesmo tropismo e que os resíduos do sítio ativo de GP63 foram mais alterados, ao contrário dos resíduos de PGFS. Também observamos que GP63 foi mais produzida na fase logarítmica dos parasitos, ao contrário de PGFS que foi mais expressa na fase estacionária. No contexto da interação parasito-hospedeiro, ainda não foi investigado a síntese de eicosanoides in situ durante a fase crônica da leishmaniose visceral (LV). Desse modo, utilizamos Hamsters Golden Syrian (Mesocricetus auratus) machos, com 6 a 8 semanas de idade, que foram infectados intraperitonealmente com 107 L. infantum/mL. Após 150 dias de infecção, avaliamos as alterações clínicas e histológicas no baço e fígado. Através da HPLC-MS/MS demonstramos pela primeira vez a produção de eicosanoides no plasma, baço e fígado dos animais. Notamos que o aumento da produção de HETEs está correlacionado com o aumento da carga parasitária sítio específica e com gravidade da LV. Portanto, este estudo abre perspectivas para compreensão da biologia de Leishmania e sua interação com o hospedeiro, bem como amplia o conhecimento para elaboração de novas estratégias antiparasitárias.

ABSTRACT Lipid droplets (LDs) are cytoplasmic organelles responsible for the storage of neutral lipids and the mediators metabolism in many types of cells. Recent data show LDs formation and eicosanoid production in Leishmania. However, there are few studies on the formation and modulation of LDs, as well as about the bioactive lipids synthesis in different Leishmania species. Thus, the formation of LDs in L. infantum, L. amazonensis and L. braziliensis, as well as the stimulation of these organelles with eicosapentaenoic (EPA), docosahexaenoic (DHA) and arachidonic (AA) acids were first evaluated using optical microscopy. The mediators produced by these species were quantified by High-Performance Liquid Chromatography - Mass Spectrometry (HPLC-MS) and the expression of glycoprotein of 63kDa (GP63) and prostagliandin F2α sinthase (PGFS) proteins were evaluated by immunoblotting and ELISA. Finally, the genomic sequences and active site residues of these proteins were analyzed in Old and New World Leishmania species. We observed a greater number of LDs during metacyclogenesis and after stimulation with EPA, DHA and AA, in L. infantum and L. braziliensis. In addition, we identified 12 bioactive lipids, mainly stimulated by AA. From the in silico analyses, we noticed that GP63 and PGFS sequences are more similar between species with the same tropism and that GP63 active site residues were more altered, unlike the PGFS residues. We also observed that GP63 was more produced in the log phase of the parasites, unlike PGFS which was more expressed in the stationary phase. In the context of the parasite-host interaction, the eicosanoids synthesis in situ during the chronic phase of visceral leishmaniasis (VL) has not yet been investigated. Therefore, we used male Golden Syrian Hamsters (Mesocricetus auratus), aged 6 to 8 weeks, which were infected intraperitoneally with 107 L. infantum/mL. After 150 days of infection, we evaluated the clinical and histological changes in the spleen and liver. Through HPLC-MS, we demonstrated for the first time the eicosanoids production in the plasma, spleen and liver of animals. We noticed that the increase in the production of the class of HETEs mediators is correlated with the inscrease of tissue-specific parasite load and severity of VL. Therefore, this study opens perspectives for understanding the biology of Leishmania and its interaction with the host, as well as expanding the knowledge for the development of new antiparasitic strategies.

RESUMO INTRODUÇÃO: Pessoas vivendo com HIV (PVHIV) frequentemente apresentam anemia como complicação. A anemia em pacientes com HIV pode ser associada a uma exacerbação do perfil inflamatório, com aumento de citocinas no sangue e progressão da doença mais acelerada. Entretanto pouco se tem relatado sobre o impacto da gravidade da anemia nos desfechos clínicos desfavoráveis durante tratamento desses pacientes com antirretrovirais, como tuberculose (TB) incidente, síndrome de imunorreconstituição inflamatória (SIRI) e morte. OBJETIVO: Caracterizar o perfil inflamatório de pacientes anêmicos com HIV, bem como identificar as associações entre a presença de anemia e sua gravidade com os desfechos desfavoráveis de tratamento desses pacientes. METODOLOGIA: Esta tese reúne um conjunto de seis manuscritos que visam caracterizar e associar a anemia aos desfechos clínicos de PVHIV. Ao todo, foram avaliados dados de 1617 PVHIV. Anemia e sua gravidade foram definidos de acordo com os critérios da Organização Mundial da Saúde. Para cada manuscrito foi utilizada uma diferente coorte de PVHIV, e em cada uma delas foram analisados os níveis plasmáticos de marcadores inflamatórios, tal como dados clínicos, socioeconômicos e laboratoriais. Foram aplicados métodos de estatística descritiva e um conjunto de técnicas multidimensionais, incluindo análises de rede, análises integrativas, regressões logísticas e cálculo de grau de perturbação inflamatória (GPI). RESULTADOS: No primeiro estudo, concluímos que PVHIV anêmicos têm maior risco de desenvolver TB incidente após o início do tratamento antirretroviral (TARV) se comparado os não-anêmicos. No segundo, foi constatado que a anemia grave aumenta o risco de SIRI, principalmente por TB, em PVHIV durante o TARV. No terceiro estudo, foi observado que a anemia está associada a uma maior disseminação da TB em PVHIV. Nos três estudos supracitados, anemia moderada/grave são fatores de risco para óbito. O quarto estudo demonstrou que baixos níveis de hemoglobina estavam associados a um maior GPI em pacientes com HIV-TB. Em seguida, observamos que pacientes HIV-TB com anemia persistente possuem maior risco de vir a óbito que aqueles não-anêmicos durante o tratamento anti-TB (TAT). Esse último resultado foi reforçado pelo sexto estudo, onde a anemia moderada/grave pré-TAT foi um fator de risco para óbito. CONCLUSÕES: Em conjunto, os manuscritos desta tese adicionam importantes informações quanto à importância da anemia no contexto da coinfecção por HIV-TB. Ao identificar a anemia moderada e grave como fator de risco para TB incidente, SIRI, TB disseminada, perturbação inflamatória e óbito, demonstramos que PVHIV devem ser cuidadosamente monitorados e, se possível, a anemia deve ser cuidadosamente avaliada como um marcador de possível infecção oportunista e pior prognóstico. Nesse contexto, essas informações são úteis no direcionamento de novas abordagens para otimizar o manejo clínicos e melhorar a qualidade e expectativa de vida de PVHIV.

ABSTRACT INTRODUCTION: People living with HIV (PLHIV) often have anemia as a complication. Anemia in HIV patients may be associated with an exacerbation of the inflammatory profile, with an increase in blood cytokines and more accelerated disease progression. However, little has been reported on the impact of anemia severity on unfavorable clinical outcomes during treatment of these patients with antiretroviral drugs, such as incident tuberculosis (TB), immunoreconstitution inflammatory syndrome (IRIS) and death. OBJECTIVE: To characterize the inflammatory profile of anemic patients with HIV, as well as to identify the associations between the presence of anemia and its severity with the unfavorable treatment outcomes of these patients. METHODS: This thesis brings together a set of six manuscripts that aim to characterize and associate anemia with the clinical outcomes of PLHIV. Altogether, data from 1617 PLHIV were evaluated. Anemia and its severity were defined according to World Health Organization criteria. For each manuscript, a different PLHIV cohort was used, and in each of them the plasma levels of inflammatory markers were analyzed, as well as clinical, socioeconomic and laboratory data. Descriptive statistical methods and a set of multidimensional techniques were applied, including network analyses, integrative analyses, logistic regressions and calculation of the degree of inflammatory perturbation (DIP). RESULTS: In the first study, we concluded that anemic PLHIV are more likely to develop incident TB after starting antiretroviral treatment (ART) compared to non-anemic individuals. In the second, it was found that severe anemia increases the risk of IRIS, mainly from TB, during ART. In the third study, it was observed that anemia is associated with a greater dissemination of TB in PLHIV. In the three aforementioned studies, moderate/severe anemia are risk factors for death. The fourth study demonstrated that low hemoglobin levels were associated with higher DIP in HIV-TB patients. Next, we observed that HIV-TB patients with persistent anemia have a higher risk of dying than non-anemic patients during anti-TB treatment (ATT). This last result was reinforced by the sixth study, where moderate/severe anemia pre-ATT was a risk factor for death. CONCLUSIONS: Together, the manuscripts of this thesis add important information regarding the importance of anemia in the context of HIV-TB co-infection. By identifying moderate and severe anemia as a risk factor for incident TB, IRIS, disseminated TB, inflammatory disturbance, and death, we demonstrate that anemic PLHIV should be carefully monitored and, if possible, anemia should be carefully evaluated as a hallmarker of possible opportunistic infection and worse prognosis. In this context, this information is useful in directing new approaches to optimize clinical management and improve the quality and life expectancy of PLHIV.

RESUMO Contexto: Hábitos alimentares não saudáveis estão relacionados ao desenvolvimento de Doenças Crônicas Não Transmissíveis (DCNT), também na população infantil. Um dos mecanismos associados é o desequilíbrio da microbiota intestinal. Com a prerrogativa de promoção de sustentabilidade social, econômica e ambiental, e de melhora das condições nutricionais e de saúde, uma intervenção que alcançou cerca de 32 mil estudantes do estado da Bahia fomentou a redução da oferta de alimentos de origem animal e ultraprocessados na alimentação escolar. O objetivo deste estudo de intervenção foi avaliar o estado nutricional desses estudantes e a sua associação com as concentrações fecais de Bifidobacterium spp. (BIF). Métodos: 190 indivíduos, de 5 a 19 anos, foram avaliados no início do ano letivo de 2019 e 124 deles, expostos à intervenção, foram novamente avaliados no final desse período. A avaliação do consumo alimentar foi feita por meio de Recordatório de 24 horas. Os parâmetros laboratoriais incluíram hemograma, perfil glicídico e lipídico, ferritina e vitaminas D e B12. A avaliação da abundância fecal de BIF foi feita pelo método de Reação em Cadeia da Polimerase em Tempo Real. Os indicadores antropométricos incluíram Índice de Massa Corporal, Circunferência da Cintura e Relação Cintura-Estatura. Resultados: Observou-se alta ingestão de alimentos ultraprocessados e alta prevalência de LDL-colesterol elevado, hipertrigliceridemia, excesso de peso e risco cardiovascular, achados mais expressivos na população de escolas da zona urbana. A menor abundância fecal de BIF foi associada à maior prevalência de hiperglicemia, e a maior concentração foi associada à menor prevalência de risco cardiovascular. A exposição à intervenção resultou em redução dos níveis séricos de LDL-colesterol e do risco cardiovascular, e aumento dos níveis séricos de triglicerídeos. Conclusão: A concentração fecal de BIF está associada com alterações metabólicas na população estudada e políticas públicas locais podem ser eficazes no enfrentamento às DCNT entre crianças e adolescentes da rede pública municipal de ensino. Sugere-se investigação da influência da prática de atividade física nesses desfechos.

ABSTRACT Context: Unhealthy eating habits are related to the development of Chronic Noncommunicable Diseases (NCDs), also among children. One of the associated mechanisms is the imbalance of the intestinal microbiota. With the prerogative of promoting social, economic, and environmental sustainability, besides improving nutritional and health conditions, an intervention that reached about 32,000 students in the state of Bahia, Brazil, promoted a reduction in the supply of animal protein and ultra-processed foods in school meals. This intervention study aimed to assess the nutritional status of these schoolchildren, and its association with the fecal concentrations of Bifidobacterium spp. (BIF). Methods: 190 subjects, aged 5 to 19y, were evaluated at the beginning of the 2019 and 124 of them, exposed to the intervention, were evaluated again at the end of this period. The assessment of food consumption was performed using a 24-hour recall. Laboratory parameters included blood count, glucose and lipid profiles, ferritin, and vitamins D and B12. The fecal abundance of BIF assessment was performed by the Real-Time Polymerase Chain Reaction method. Anthropometric indicators included Body Mass Index, Waist Circumference, and Waist-to-Height Ratio. Results: A high intake of ultra-processed foods and a high prevalence of hypercholesterolemia, hypertriglyceridemia, overweight and cardiovascular risk were observed, the most expressive findings in the population of urban schools. The lower fecal abundance of BIF was associated with a higher prevalence of hyperglycemia, and a higher concentration was associated with a lower prevalence of cardiovascular risk. Exposure to the intervention resulted in reduced serum LDL-cholesterol levels and cardiovascular risk, and increased serum triglyceride levels. Conclusion: The fecal concentration of BIF is associated with metabolic changes in the studied population and local public policies can be effective in fighting NCDs among children and adolescents in the municipal public school system. It is suggested to investigate the influence of physical activity on these outcomes.

INTRODUÇÃO: A síndrome inflamatória de reconstituição imune associada à tuberculose (TB-SIRI) é um agravamento clínico dos sintomas de tuberculose observados em uma fração de pacientes coinfectados com HIV logo após o início da terapia antirretroviral (TARV). É bem conhecido que a TBIRIS ocorre em função de inflamação exacerbada e dano tecidual em resposta à produção elevada de IFN-γ derivado de células T CD4+. Concomitante a reconstituição linfócitaria, diversas alterações em diferentes camadas de organização biológicas são alteradas e dão suporte ao fenomenos patologicas associados a SIRI. Sabe-se que perturbações metabólicas subjacentes às alterações qualitativas das funções de diversas células imunológicas culminam na exacerbação da inflamação sistêmica e hiperativação celular. Além disso, vários estudos destacam o papel das células imunes adaptativas na patogênese da IRIS, mas até que ponto a ativação dos linfócitos T contribui para o desenvolvimento da SIRI ainda precisa ser esclarecido. OBJETIVO: Caracterizar a ativação e perfil de memória de linfócitos T CD4+ e CD8+ em indivíduos TB coinfectados com HIV mediante ao início do tratamento antirretroviral (TARV). Juntou-se a essa tese outro manuscrito que investigou a aplicação de técnicas de multi-ômicas para a bioprospecção de marcadores associados a TB-SIRI. MÉTODOS: Este foi um estudo retrospectivo de pacientes TB-HIV do sul da Índia antes e semanas após o início do TARV, em que houve a caracterização fenotípica dos linfócitos e determinação do grau de inflamação sistêmicas nestes pacientes. RESULTADOS: Observamos que a SIRI está relacionada com alterações no metabolismo de aminoácidos e lipídios. Tais vias metabólicas foram correlacionadas com mediadores pro-inflamatórios elevados durante a SIRI. O segundo manuscrito desta tese revelou que pacientes que desenvolveram SIRI possuíam uma linfopenia de células T CD4+ acentuada antes do início da TARV e possuíam elevadas frequências de células T CD8+. Além disso, os episódios de IRIS foram associados às disfunções da dinâmica de ativação linfocitária, com elevação de linfócitos T CD4+ HLA-DR+ e células T CD4+ e CD8+ expressando granzima B. Desenvolvemos modelos baseados em algoritmos de aprendizado de máquinas que foram capazes de predizer e diagnosticar IRIS levando em consideração as frequências de linfócitos expressando moléculas associadas a ativação celular. No terceiro manuscrito, observamos que a reconstituição do compartimento de células T CD8+ de memória é predominada por células de memória efetora em pacientes que desenvolvem SIRI. Por sua vez, a magnitude da variação das frequências dessas células esta correlacionada positivamente com as abundancias de citocinas pro-inflamatórias. Em pacientes que manifestaram SIRI associada a TB, ocorre retenção de células T CD8+ expressando CXCR3 em sítios persistentemente inflamados e a frequência destas células foi capaz de distinguir os pacientes com grande acurácia. CONCLUSÃO: Os dados apresentados nesta tese ratificam o papel da ativação linfocitária na imunopatogênese da SIRI, sendo que os subconjuntos destas células e o perfil de alteração metabólicas são capazes de diagnosticar ou predizer a manifestação desta síndrome.

INTRODUCTION: Tuberculosis-associated immune reconstitution inflammatory syndrome (SIRI) is a clinical picture of tuberculosis symptoms observed in a disease of patients co-infected with HIV shortly after initiation of antiretroviral therapy (ART). It is well known that TB-IRIS occurs as a function of exacerbated inflammation and tissue damage in response to the overproduction of IFN-γ derived from CD4+ T cells. Concomitant to several lymphocyte reconstitution, changes and different layers of biological organization as alterations and support to the pathological phenomena associated with SIRI. Several immunological studies stand out, but that contribute to the great evolution of the role of genetics in the development of T lymphocytes, but that contribute to the great evolution of the development of T lymphocytes, remains highlighted. OBJECTIVE: To evaluate the lymphocyte profile of patients starting antiretroviral treatment. This thesis was joined by another manuscript that investigated the application of multi-omics techniques to bioprospecting for indicators associated with TB-SIRI. METHODS: This was a retrospective study of TB-HIV patients from South India before and weeks after initiation of ART, in which there was a phenotypic characterization of lymphocytes and determination of the degree of systemic inflammation in these patients. RESULTS: We observed that SIRI is related to alterations in amino acid and lipid metabolism. Such metabolic pathways have been correlated with elevated pro-inflammatory mediators during SIRI. The second manuscript of this thesis revealed that patients who developed IRIS had marked CD4+ T-cell lymphopenia before starting ART and had high frequencies of CD8+ T-cells. In addition, IRIS episodes were associated with dysfunctions of lymphocyte activation dynamics, with elevation of CD4+ HLA-DR+ T lymphocytes and CD4+ and CD8+ T cells expressing granzyme B. We developed models based on machine learning algorithms that were able to predict and diagnose IRIS taking into account the frequencies of lymphocytes expressing molecules associated with cell activation. In the third manuscript, we observed that the reconstitution of the memory CD8+ T cell compartment is predominated by effector memory cells in patients who develop IRIS. In turn, the magnitude of the frequency variation of these cells is positively correlated with the abundance of pro-inflammatory cytokines. In patients who manifested TB-associated SIRI, there is retention of CD8+ T cells expressing CXCR3 in persistently inflamed sites and the frequency of these cells was able to distinguish patients with great accuracy. CONCLUSION: The data presented in this thesis confirm the role of lymphocyte activation in the immunopathogenesis of IRIS, and the subsets of these cells and the profile of metabolic alterations are capable of diagnosing or predicting the manifestation of this syndrome.
