SUMMARY:

There are different care policies for people who use Psychoactive Substances (PAS). In Brazil, the asylum models, which start from the perspective of abstinence, currently coexist with the public health model, which has Harm Reduction as its main guideline. The latter, inserted in the Unified Health System, uses the Psychosocial Care Centers (CAPS) as its main monitoring service for people who use drugs. On the other hand, from the perspective of abstinence, there are the Therapeutic Communities (CTs) which, with advances in recent years, have become the dominant space for asylum treatment. Considering this context, the principal objective of this study was to analyze the effects of the experience of stay in long-term institutions for people who use drugs. As a specific objective, the implications of the hospitalization process with regard to social ties were investigated. To achieve the proposed objectives, semi-structured interviews were carried out with six people who experienced some period of hospitalization in at least one Therapeutic Community or similar institution. As an analysis method, we chose discourse analysis (AD) based on the contributions of Michel Foucault. This research, developed within the scope of a professional master's degree in Health Psychology, dialogues directly with the line of research Clinical Practices and Mental Health, and is justified by the potential to put into discussion and implement practices in the field of health psychology and in dialogue with the national reality. In the first article presented, aiming to support empirical research, an integrative literature review was carried out, with CTs as the main focus of analysis. The search was carried out in the Lilacs and Medline databases, and a total of 28 articles were selected for detailed analysis. We recognize contradictory statements regarding the origin, concept, definition and functioning of CTs, as well as inaccuracies regarding the standardization of the care offered. The results exposed serious weaknesses in guaranteeing the health security of these institutions. A practical difficulty was observed in standardizing and regulating CTs. In the second article presented, the aim was to carry out a theoretical-critical analysis of the discourses that circulate regarding people who use drugs and their relationship with neoliberal rationality. As an epistemological and theoretical foundation, we rely on the analyzes proposed by Michel Foucault regarding medical, legal and moral discourses. It was understood that the neoliberal context has intensified inequalities and social exclusion, also affecting the way in which policies for people who use drugs are developed. In the third chapter we present the results of the research carried out. From the analysis of the interviews, we discuss the discursive manifestations and the way in which they intertwine and influence each other with regard to policies for people who use drugs. The prohibitionist practices of therapeutic communities are regulated by standardizing conduct, updating what is considered normal, desirable, moral and legal, and reinforcing what is seen as abnormal, undesirable, immoral and illegal. The reproduction of prohibitionist discourses is fueled by the existence of segregation spaces and, in the case of CTs, the introduction of the Christian-moral foundation as a basis for treatment was noticed, legitimizing much of their conduct. Finally, the effects were the reinforcement of social practices of exclusion, silencing and stigmatization on subjects and groups that do not meet certain performance standards. From listening to people who went through the hospitalization experience, a technical-literary work was produced as a final product. Structured in the form of short texts, with a poetic-psychoanalytic tone, the work was made in search of creating that which cannot be represented directly.
